| II | Optic Sight (light, shade, perception of objects) -"Do you wear contacts/glasses? Do you have any loss of vision, blurred vision, spots?" -Patient holds hand over one eye, holds something to read at arms length -Vision fields test |
| III | Oculomotor Move and blink eyes -Shine light in eye, pupils -Eyes follow finger in all four quadrants, look for nystagmus, finger to nose, cross eyed, etc. -finger from nose to examiner finger (accuracy only important) |
| IV | Trochlear Move eyes up, down, back, forth -III, IV, VI tested the same |
| V | Trigeminal Sensation of forehead/face, jaw function -Stroke forehead, cheeks, jawline, clench jaw |
| VI | Abducens Eye movement -III, IV, VI tested the same |
| VII | Facial Facial muscles/expressions, taste -Wrinkle forehead, close eyes tight, smile and show teeth, frown Check of symmetry |
| VIII | Acoustic Hearing and balance -Close eyes, examiner rubs fingers near ear |
| IX | Glossopharyngeal Sensation of upper mouth/throat area, gag reflex, constriction of pharyngeal wall -Not tested |
| X | Vagus (Many functions) Control of roof of mouth, vocal cords -Open mouth, say "ahhh". Note for hoarseness. Watch palate rise/fall |
| XI | Spinal Accessory Turning of head, shrugging of shoulder muscles -turn head against resistance, shrug shoulders against resistance |
| XII | Hypoglossal Tongue movement -stick tongue out, move up, down, side to side Check for tongue twisting to affected side |
