| Question | Answer |
|---|---|
| What are the types of clamps used in obstetrics? | Dunhilde, Green Armytage, Heaney, Kocher's, Littlewood's, Uterine artery |
| What are the uses of the Dunhilde clamp? | Haemostasis, Holding Sutures, Marking gauze packs used to pack away bowel |
| What are the uses of the Green Armytage? | Haemostasis. Especially at angles of uterine incision in Caesarean section |
| What are the uses of the Heaney Clamp? | Clamping uterine pedicle |
| What procedure is the heaney clamp usually used in? | Hysterectomy |
| What procedure is the Green Armytage known for? | Caesarean Section |
| What is the use of the Kochker's Clamp? | Holding fibrous tissue such as rectus sheath and ligaments |
| What are procedures in which Kochker's is used? | Amniotomy, Abdominal hysterectomy |
| What are the uses of surgical clamps? | Haemostasis, holding structures |
| What are the uses of the Littlewood's clamp? | Grasp cervix in D&C, cervical cerclage, cone biopsy, vaginal hysterectomy |
| What procedures are the Littlewood's used in? | D&C, cervical cerclage, vaginal hysterectomy, cone biopsy |
| What are the uses of the uterine artery clamp? | Clamp 1. uterine arteries, 2. cardinal ligaments during hysterectomy |
| What is the procedure in which the uterine artery clamp is used? | Hysterectomy |
| What is the difference between the uterine artery clamp and the Dunhilde (appearance)? | Uterine artery clamp has longer jaws |
| What are the different types of forceps? | Allis, babcock, Dartiques, Dissecting, polypectomy |
| What is the function of the allis forceps? | Grip tissue with minimal trauma ex. skin. Used for slippery or dense tissue such as breast, vagina, thyroid |
| In what procedures are allis forceps usually used? | In almost all procedures |
| What is the function of the Babcock forceps? | Used to hold delicate tissue with no damage, particularly fallopian tubes. Less traumatic than Allis forceps. |
| What procedures may the Babcock forceps be used in? | Ex. tubal ligation, salpingectomy |
| What is the function of the Dartigues forceps? | Hold larger structures atraumatically ex. uterus Elevates vaginal wall in order to remove polyps, tissue or tumours from uterus |
| What is a procedure in which the Dartigues is used? | Myomectomy |
| What is the function of the dissecting forceps? | Hold tissue during dissections |
| What procedure may dissecting forceps be used? | Anything ex. hysterectomy, c-section, myomectomy |
| What is the function of polypectomy forceps? | Removal of endocervical or uterine polyps |
| What are the different types of retractors in OBGYN? | Doyen's, Kirschner's, Dever's, Balfour, Czerney, Morris, Cusco's. Grave's, Sim's |
| What are the uses of Dever's retractors? | Hold bowel away during pelvic surgery. May be used in both abdominal/pelvic surgery |
| What is the use of the Doyen's? | retract bladder during C-section |
| Use of Balfour? | Retract anterior abdominal wall during pelvic surgery (ex. hysterectomy) |
| Use of Kirschner? | Retract incision of abdominal wall in 4 directions |
| Use of Morris? | Retract ant abdominal wall during pelvic/abdominal surgery (esp during opening and closure) |
| Czerney use? | Retract SKIN of ant abd wall to expose rectus sheath underneath |
| cusco speculum function | visualise cervix and lateral vaginal walls |
| procedures in which cusco can be used | pap smear, cervical biopsy, endometrial aspiration, high vaginal swab, endocervical swab |
| equipment needed to pass a cusco speculum | cusco speculum, lubricant, cytobrush (if doing pap smears), swabs if investigating for bacterial infection, pipelle. gloves |
| how would you prepare a patient for a speculum examination? | Introduce self, explain procedure and need for procedure, obtain verbal consent, ensure chaperone present |
| how would speculum examination be performed? P1 | 1 Perform abdominal exam. 2 Lithotomy position &externally examine genitalia 3 Observe for prolapse by asking pt to cough 4 Lubricate speculum & inform pt 5 pass spec w/ RH while holding open labia with LH w/ blades 90deg |
| how would speculum examination be performed? P2 | 6 Rotate so blades r 180 deg w/ screws upwards 7 slowly open blades & view cervix 8. tighten screw to hold spec in place if performing further procedure 9 remove spec by undoing screw, letting blades close lightly rotating back 90 deg and removing |
| how would speculum examination be performed? P2 | 10 thank pt, wash hands |
| what is grave's speculum | speculum with protruding inferior blade |
| diff between cusco and grave's | grave's opens in 2 planes |
| special use of grave's | colposcopy |
| special use of sim's spec | diagnose prolapse, d&c, hysteroscopy |
