coding conventions

coding conventions general rules used in the classification and they are independent of the coding guidelines
abbreviations to save space
NEC ( not elsewhere classifiable) means other or other specified
NOS (not otherwise specified) equivalent of unspecified
and appears in category titles and code descriptions in icd-10-cm tabular list of diseases and injuries, icd-10-pcs index, icd-10-pcs tables, interpreted as and/or
boxed note define terms and provide coding instructions
code also alerts the coder that more than one code may be required to fully describe the condition.
cross-reference used in icd-10-cm and icd-10-pcs, instructs the coder to refer to another entry in the index or tabular list to assign the correct code.
see instructions after a main term directs the coder to another term in icd-10-cm and icd-10-pcs indexes to locate code.
see also located after main term or subterm in the icd-10-cm indexes and directs the coder to another main term or subterm that may provide useful index entries
see category directs coder to the icd-10-cm tabular list where a code can b selected from the options provided there
see condition directs coder to the main term for a condition, found in icd-10-cm disease index
default code code listed next to the main term in the icd-10-cm indexes
due to located in icd-10-cm index to diseases & injuries, alphabetical order to indicae the presence of cause and effect relationship between two conditions.
eponym disease or syndrome that are named for people
etiology and manifestation rules icd-10-cm tabular list of diseases and injuries, code first underlying disease, code, if applicable, any casual condition first, use additional code, in diseases classified elsewhere

other code is assigned when patient record documentation provides detail for which a specific code does not exist in icd-10-cm

punctuation brackets, colons, parentheses
brackets identify manifestation codes in the icd-10-cm index to diseases and injuries
colon used after incomplete term or phrase to make the code assignable to given category
parentheses enclose nonessential modifiers, used in icd-10-cm and icd-10-ocs indexes
syndrome association of clinically recognizable features such as signs, symptoms, and/or characteristics that often occur
tables organize subterms, 2nd & 3rd qualifiers, their codes in columns and rows to make it easier to select the proper code
trust the index a concept that that icd-10-cm disease index containing many diagnostic terms that do not appear in tabular list
typeface for ease in reference. bold face, italicized types.
unspecified code assigned because patient record documentation is insufficient to assign a more specific code.
use directs coder to a body part or device in icd-10-pcs index
with located immediately below the main term, not in alphabetical order. appears in icd-10-cm index to diseases and injuries, physician must document both presence of conditions or procedures in patients record
