Coding conventions for ICD -10-CM and ICD -10-PCS Are general rules used in the ICD -10-CM classification system that are independent of coding guidelines
Default code ICD -10-CM code listed next to a main term in the index.....represents that condition that is most commonly associated with the main term,or is the unspecified code for the condition
Syndromes Are clinically recognizable features such as signs(observed by others)symptoms (reported by the patient)and characteristics that often occur together
Eponyms Are diseases or syndromes that are named for people... listed in alphabetical sequence as main terms,they are listed as subterms below the main terms,a description of the disease syndrome or procedure is usually included in parentheses following the epony
NEC Not elsewhere classifiable...means "others" or "other specified "and identifies codes that are to be assigned when information needed to assign a specific code can't be located
NOS Not otherwise specified..it identifies codes that are to be assigned when information needed to assign a more specific code cannot be obtained from the provider.
Brackets Are used to identify manifestation codes
Manifestation Is a condition that occurs as the result of another condition..manifestation codes are reported as secondary codes

If an index entry is too long to fit on the line,additional text is Printed on the next line and indented five spaces
Each ICD-10-CM and ICD -10-PCS index uses an indented format for ease in reference All ICD -10-CM index to disease and injuries subterms associated with main terms are indented two spaces with 2nd an 3rd qualifiers with the main term indented by 2 and 4 spaces,if a index require more than 1 line the text is printed on the next line and
ICD-10-CM and ICD -10-PCS also makes use of the boldface and italic typeface for ease in reference Boldface type is used for main entries alphabetical indexes,all codes and descriptions in the tabular list, italicized type is used for tabular list exclude1 an exclude2 to identify manifestation codes
ICD -10-CM included the following punctuation Bracket,colons,and parentheses
Parentheses are used in both ICD -10-CM index and tabular list ICD -10-PCS index to enclose nonessential modifiers which are supplementary words that maybe present or absent for physicians notes
Colon Used after an incomplete term or phrase in the ICD -10-CM disease index and tabular list
Modifiers (additional terms) Following the colon is needed to make the condition assignable to a given category
Tables Organize subterms 2nd qualifiers and 3rd qualifiers,their codes in columns and rows to make it easier to select the proper code
Neoplasm is an alphabetical index of anatomical sites,there is 6 possible codes which is.... Malignant, benign,in situ,of certain behavior, or of unspecified nature
