What kind of prescriptions can be accepted from out of state? Non-CS from MD, DO, or DDS (or the PA AND NPs acting under their delegation); CS from MD,DO or DDS if ok to do so in that other state.

How long must a wholesale distributor keep record of outdated drugs? 2 years after the disposition of the outdated drugs
What is a "wholesale distributor"? If the number of dosage units of all Rx drugs distributed by a pharmacy is greater than 5% of total dosage units distributed and dsipensed byt he pharmacy in a 12 month periord.
When is a pharmacy considered a Manufacturer? If the total number of dosage units of all Rx drugs that are prepared or coumpounded by a pharmacy for resale by another person during a consecutive 12 month period is more than 5% of the total number of units of drugs prepared by the pharmacy.
What is the max fine by the board? $5,000 per violation
What are the CE requriements for MI? 30 HRS BIENNUAL; 10hr ce live; 1hr pain management; max 12 hrs ce 1 day;
What is the pharmacist to intern ratio? 2 interns per pharmacist
What are the requirements to be a preceptor? must be engaged in practice of pharmacy for at least 1 year
What reqirements are set for radiopharmaceuticals? Pharmacy area must occupy at least 300 square feet of space; and a storage/decay area no less than 25 square feet of space.
What is positive identification? includes a photograph and dob
What is readily retrievable? can be separated from other files within 48 hours; for CS HAS AN ASTERISK; REDLINED; OTHER MANNER VISUALLY ID'ABLE FROM OTHER RECORDS (STAMPED SIGNATURE IS INVALID FOR CS)
How long must you maintain a CS rx? 5 years from the last date of dispensing
What 8 things are needed on a CS rx? name/address of pt; DEA #, Dr's name/address/ professional designation; drug name/strength/form; drug qty; sig; date; signature of Dr
What add info do you need for phone orders of CS 3-5? name of transmitting agent and name of who received it at the pharmacy
What extra is needed on a electronic transmission of rx? name address of DR; e-signature; Dr's phone; Time/date of transmission; name of the pharmacy intended for receiving the transmission
What is required in emergency c-d dispensing? all the regular stuff except dr signature & dr must give you a rx that states "authorization for emergency dispensing" within 7 days
When can you partial a C2? if unable to supply all and can in 72 hours; or LTC facility or terminally ill (qty must be less than 60 days)
What must be recorded for partial C2s? date of partial; qty dispensed; qty remaining; who the rph was
What limits are set on C5s w/o rxs? max 8 oz/ 48 doses of opium; max 4 oz/ 24 doses of all other C5s in any 48 hr period
What are the transfer rules? CS 3-5 only can transfer once (unless real-time computer system; then as many times as authorized); no C2 transfer; must right VOID on rx (record name/address/ dea # of pharmacy; name of the pharmacist who took and gave transfer)
MAPS (Michigan automate Prescription system) you must report what? Pt identifier (id number); CS name; qty dispensed; ndc; date of issue; date of dispensing; day supply; rx number; dr/pharmacy DEA #; MI license # of dispensing RPh.
What is ASAP? American society of automation in pharmacy
When do you have to report to MAPS? TWICE monthly (by the first calendar day and the 15th calendar day of each month)(can report w/in 2 days of the 1st and 15th)
When do you not have to report to MAPS? When CS is administered directly to the pt; when CS is dispensed to a health care facility or agency by a dispensing prescriber in qty adequate to treat not more than 48 hours.
What are ephedrine regulations for OTC sale? max 400 mg guaifensin and 25mg ephedrine per dose; blisters not more than 2 tabs/caps per blister; max 12g possed by any person; Max dose is 100mg/qd
How do you handle the loss/theft of a CS? Within 10 days must notify the administrator w/ a DEA for 106.
How must records for CS be kept? retrievable in 48 hrs & kept for 2 years
What are inventory requirements? C1-2: exact ct; C3-5: estimate unless container has more than 1000. Taken and open/close of business; maintain invetory in writting; sign/date;licensee's name/address/dea # on inventory; c2 on separate page; annually; keep and make available for inspectio
How must CS be filed? C2 separate; C3-5 together
Prescriptions must be maintained for how many years? 5 years

What are drug interchange rules in MI? non AB product interchange is ok as long as they are generic equivalents of each other; caution w/ narrow therapeutic ranges
What prescriptions can you not fill from out of state? Ones written by DVMs, Podiatrists; optometrists (unless under supervision of a MD/DO)
What is a RPh required to do for the first fill of a RX at the pharmacy? Rx must be numbered; dated; initialed by RPh
How long are non CS Rxs valid? 1 year from issued date
What information is required on a Rx label (community)regarding name of the medicaiton? Both the name of prescribed and dsipensed; no brand name then brand prescribed &generic name; if a brand name selection the maufacturer/supplier will be noted; if written generic/dispense is dif. from brand prescribed then manu/supplier must be on lable

When is a copy transfer not needed? When it is between linked chain pharmacies (this constitutes one pharmacy)
What must be on a manufacturers label other wise considered misbranded? name/place of business of manufacturer or name/place of business of packer/distributor
What is a CPMP? Customized pt medication packages; prepared by a RPh for pt and contains 2 or more prescribed solid oral dosage forms
What is required for a CPMP? label; pt package insert; non reclosable; can not be returned to stock/ dispensed to other patient
What 8 things are on a CPMP label? day/time period; serial # of CPMP and each drug; name;strenth;description/quantity of each drug; name of dr for each drug; sig; date prepared; expriation date; name/address/phone of dispenser
How do you set an expiration date of a CPMP? Earliest manufacturing date or 60 days after dispensing
What record keeping is required for CPMPs? pt name address; serial number of rx number; info on characteristics of the CPMP; date of preparation; expiration date; labeling instructions; pharmacists inital/name
What are the restocking rules for medical institutions? meds in single-unit packages and IV soln that are tamper evident may be restocked; and med that leaves the medical institution may not be restocked
How must medications be ID'ed in a medical institution? Name/strength labeled on container dispensed/each unit; name of pt and ID # on med container (container may be kept in pt individual med drawer; sig must be on container)
How must meds be kept in the absence of a RPh? locked cabinet (limited access; eeach med labeled;written order/proof of removal/use doc need be submitted; pharmacist must audit Q30 days; RPh must meet with P&T quarterly

When is a drug control license not needed? if dispensing in an ER; trauma center; or only issuing complementary starter dose
How can a RPh legally distribute to a MD? Can distribute a control soln at no more than 1 ounce at a time; contain no more than 20% narcotic substance; which is admin to patients by MD
Why cant MDs write rx of CS for the purpose of office use? its considered to be distribution
What rules govern electronic transmission of rx (community)? must have all required info; RPh must exercise professional judgement; Rx must be transmitted to the pharmacy of pt choice and only at the option of the pt.
What must a C5 label (w/o rx) have on it? date/ his or her name/ name, address of dispensed location
