Level 1 - Motor Response | No motor response |
Level 2 - Motor Response | Extension to pain - decerebrate response |
Level 3 - Motor Response | Flexion in response to pain - decorticate response |
Level 4 - Motor Response | Withdrawn from pain - pulls part of body away when pinched |
Level 5 - Motor Response | Localizes to pain - purposeful movements towards changing pain stimuli |
Level 6 - Motor Response | Obeys commands - the person does simple things as asked |
Level 1 - Verbal Response | No verbal response |
Level 2 - Verbal Response | Incomprehensible sounds -- moaning but no words |
Level 3 - Verbal Response | Inappropriate words - random or exclamatory speech, but not in a conversational exchange |
Level 4 - Verbal Response | Confused - patient responds to questions coherently but there is some disorientation and confusion |
Level 5 - Verbal Response | Oriented - patient responds coherently and appropriately to questions such as the patient's name, age, where they are, year, etc. |
Level 1 - Eye Response | No eye opening |
Level 2 - Eye Response | Eyes opening in response to pain |
Level 3 - Eye Response | Eyes opening to speech |
Level 4 - Eye Response | Eyes opening spontaneously |
