Congestive Heart Failure classes and exercise guidelines

Class I | Mild CHF; no limitation in physical activity (up to 6.5 METs); comfortable at rest, ordinary activity does not cause undue fatigue, palpitation, dyspnea, or anginal pain. |
Class II | Slight CHF; slight limitation in physical activity (up to 4.5 METs); comfortable at rest, ordinary physical activity results in fatigue, palpitation, dyspnea, or anginal pain. |
Class III | Marked CHF; marked limitation of physical activity (up to 3.0 METs); comfortable at rest, less than ordinary activity causes fatigue, palpitation, dyspnea, or anginal pain. |
Class IV | Severe CHF; unable to carry out any physical activity (1.5 METs) without discomfort; symptoms of ischemia, dyspnea, anginal pain present even at rest, increasing with exercise. |
Exercise guidelines | Heart rate limited to resting HR + 10-20 bpm; exercise HR > 115 bpm generally contraindicated. Patients with CHF (capabilities under 6 METs) are not candidates for resistance training. |
