Obtaining Preassessment Information

case history | starting point in understanding our client. filled out by client/caregiver and reviewed before the initial meeting. |
intake interview | sometimes the first face to face interaction with the client/caregivers. consists of 3 phases- the opening, the body, and the closing. |
opening phase | make introductions, describe the purpose of the meeting, and explain the structure of the meeting. |
body phase | mostly intake interview. major content areas discussed. clarify and confirm relevant info from the case history form. discuss history and current status in depth. focus on comm. development, abilities, problems/concerns. clarify other pertinent info. |
closing phase | summarize major points from the body of the interview, express appreciation for the interviewee's help, indicate next steps that will be taken. |
open ended | it is generally best to begin the interview with these types of questions because it will help identify primary concerns that often require further clarification and follow up. |
true | t/f: you should ask general questions (please describe your concern, when did you 1st notice this, how did it begin, gradual, sudden?) and then you can ask more specific questions tailored towards the presented disorder |
articulation | these are questions specific to what? how long have you been concerned about your speech? how well does your family understand you? do they ask you to repeat yourself? how well do friends and acquaintances understand you? |
language (child) | these are questions specific to what? who does the child interact w/ most often? does your child seem to understand you/others? how well do you understand your child? does your child maintain eye contact? approx. how many words does child use/understand? |
stuttering | these are questions specific to what? when did you first begin to stutter? who noticed it/what were speaking situations? does anyone else in fam. stutter? who? how do friends/fam react to the stuttering? what situations do you stutter the least/most? |
voice | these are questions specific to what? describe your voice when the problems first occurred? how has it changed over time? what do you think the cause is? do you ever run out of breath when you talk/describe it? what situation is it the best/worst? |
language (adult) | these are questions specific to what? do you have issues in your native lang and english? do you read/how often? in school did you have any problems learning? have you had any accidents/illnesses that have affected your lang? |
privacy rule | apart of HIPAA. states that it is not legal to request info about a client that is not reasonably necessary for the client's care nor is it appropriate to provide protected info to another provider that is not reasonably necessary. |
