Olfactory Nerve -Cranial 1 nerve patient has to identify a smell with each nostril seperatley and eyes closed.
Optic nerve -Cranial 2 patient reachs from material held 36cm away. Should be able to read with both eyes
Oculomotor: Reaction to light -Cranial 3 aproach patient from side with a penlight and shine on the pupil. repeat
Oculomotor: Accomodation look at near object then at distsnt. Alternate between the two and bring the object towards the nose. Pupils should contrict when obejct near and dilate for near obects. Pupil should constrict when object near the nose
Trochlear Cranial 4 Penlight 1ft infront of pt eyes. Follow movement
Cranial 5: Trigeminal Client looks up: touch slera of eye- blink reflect Light sensation: close eyes and wipe cotton over forehead Deep sensation: alternating blunt and sharp ends of object. Warm and cold objects.
Cranial 6: Abducens Penlight 1ft infront of pt. follow movement through 6 cardinal fields
Cranial 7: Facial Smile, raise eyebrows, frown, puff out cheeks, close eyes tightly. Identify different tastes.
Cranial 8: Vestibulocochlear Occlude 1 ear and out of sight place watch 2-3cm and ask for sound. Walk across room, asses gait
Cranial 9: Glossopharyngeal Say ah, yawn. Observe soft palate movement Elciti gag response Note swallow ability
Cranial 10: Vagus Swallow and speak note hoarseness
Cranial 11: Accessory Shrug shoulders against resistance from hands. Turn head side to side against resistance
Cranial 12: Hypoglossal Protrude tongue at midline and move side to side
