Cleft Palate Exam 2

Cleft lip feeding Rarely and issue, sometimes with first latching but actual feeding issues are minimal
What causes cleft palate feeding issues? Negative pressure, gap makes it difficult to create suction
What can be done to reduce nasal reflux and improve feeding? Selection of bottle/nipple, positioning of nipple. positioning infant, follow feeding schedule, frequent burping
Weak suck leads to Increased feeding time
Increased feeding time leads to insufficient weight gain
Insufficient weight gain leads to Nasal reflux
Nasal reflux leads to Fatigue
Fatigue leads to Parental stress
Parents with cleft babies Should be monitored for symptoms of stress/postpartum
Recommendation for moms during feeding Follow baby lead, keep feed diary, limit feeding times to 30 minutes, smaller more frequent feedings
What should be encouraged? Nonnutritive sucking
Types of assisted milk delivery Haberman Bottle, Pigeon Nipple, Mead Johnson Nurser
Major disadvantage of assisted milk delivery Cost
Haberman One-way valve, feeder activated with tongue pressure, immolates mechanism in breastfeeding rather than by sucking
Pigeon Nipples Not assisted delivery, firm side to occlude palate and direct milk down, soft side easier for babies tongue to push
Mead Johnson Cross-cut soft nipple, parent control feeding, longer nipple than others (affect cleft), sometimes has leakage at collar, goof for hypotonia and weakest babies
Other options Pigeon valve placed in standard nipples, cross-cut nipples, squeezing drop ins to assist with extraction (syringe)
When should children w/ cleft transition to solids? Same time as TD (4-6 months)
