Postural Drainage

NPTE: Postural Drainage Positions (scorebuilder 2008/pulmonary class notes)

Adult Upper Lobes: Apical Segments Bed or table flat. Patient leans back on pillow at 30 deg angle against therapist. Therapist claps between clavicle and top of scapula on each side
Adult Upper Lobes: Posterior Segments Bed or drainage table flat. Patient leans over folded pillow at 30 deg angle. Therapist stands behind and claps over upper back (shoulders) on both sides.
Adult Upper Lobes: Anterior Segments Bed or drainage table flat. Patient lies on pack with pillow under knees. Therapist claps between clavicle and nipple on each side.
Adult Right Middle Lobe Foot of table elevated 16". Patient lies head down on left side and rotates 1/4 turn backward. Knees flexed. Therapist claps over right nipple area.
Adult Left Upper Lobe: Lingular Segment Foot of table elevated 16". Patient lies head down on right side and rotates 1/4 turn backward. Knees flexed. Therapist claps over left nipple area.
Adult Lower Lobes: Anterior Basal Segments Foot of table elevated 20". Patient sidelying, head down, pillow under knees. (lie on right to drain left, lie on left to drain right). Therapist claps over lower ribs.
Adult Lower Lobes: Lateral Basal Segments Foot of table elevated 20". Patient lies prone, head down, then rotates 1/4 turn upward. (to drain right segment, turn up to the right). Upper leg is flexed over a pillow for support. Therapist claps over uppermost portion of lower ribs.
Adult Lower Lobes: Posterior Basal Segments Foot of table elevated 20". Patient lies prone, head down, with pillow under hips. Therapist claps over lower ribs close to spine.
Adult Lower Lobes: Superior (Basal) Segments Bed or table flat. Patient lies prone with two pillows under hips. Therapist claps over middle of back at tip of scapula on either side of spine.
Child Upper Lobes: Apical Segments Prop child on sofa with pillows, child should be in between sitting and lying (45 deg). Clap between clavicle and top of scapula.
Child Upper Lobes: Posterior Segments Lean child forward on pillow (45 deg), while sitting on your lap. Clap over upper back (shoulders).
Child Upper Lobes: Anterior Segments Lie child supine on flat surface. Clap between clavicle and nipple.
Child Right Middle Lobe Sit on chair with legs straight out. Child on lap, rolled to left side, head down. Clap over right nipple area.
Child Left Upper Lobe: Lingular Segment Sit on chair with legs straight out. Child on lap, rolled to right side, head down. Clap over left nipple area.
Child Lower Lobes: Anterior Basal Segments Sit on chair with legs straight out. Child on lap, supine, head going down your legs. Clap over lower ribs.
Child Lower Lobes: Lateral Basal Segments Sit on chair with legs straight out. Child on lap, rolled to side (left sidelying to drain right side), head down, (more incline than with middle and lingular segments). Clap over lower ribs.
Child Lower Lobes: Posterior Basal Segments Sit in chair with legs out, lie child prone on your legs. Clap over lower ribs.
Child Lower Lobes: Superior (Basal) Segments Sit on sofa. Lie child prone across lap. Clap over middle of back at tip of scapula.
