INDICATIONS1. Hypoglycemia, 2. Hyperkalemia 3. Magnesium sulfate overdose 4. Calcium channel blocker overdose 5. Adjunctive therapy in treatment of insect bites and stings
CONTRAINDICATIONS1. Hypercalcemia 2. V-fib 3. Digitalis toxicity
ADVERSE REACTIONS1. Bradycardia, 2. Asystole. 3. Hypotension 4.Peripheral Vasodilation. 5. Metallic taste. 6. Local necrosis. 7. Coronary and cerebral artery spasm. 8. Nausea and Vomiting.
NOTES ON ADMINISTRATIONPregnancy safety Category C. Do not use routinely in cardiac arrest.
INCOMPATABILITIES/DRUG INTERACTIONSMay worsen arrhythmias secondary to digitalis toxicity. May antagonize the effects of verapamil. Do not mix with or infuse immediately before or after sodium bicarbonate without intervening flush.
DOSE/ROUTE Adult 500-1000mg (5-10 ml of a 10% solution) IV/IO push for Hyperkalemia and calcium channel blocker overdose. May be repeated as needed. DOSE/ROUTE Pediatric
ONSET OF ACTION5-15 minutes
PEAK EFFECTS3-5 minutes.
DURATION OF ACTION15-30 minutes, but may persist for 4 hours (dose dependent).
