INDICATIONS | 1. Hypoglycemia, 2. Hyperkalemia 3. Magnesium sulfate overdose 4. Calcium channel blocker overdose 5. Adjunctive therapy in treatment of insect bites and stings |
CONTRAINDICATIONS | 1. Hypercalcemia 2. V-fib 3. Digitalis toxicity |
ADVERSE REACTIONS | 1. Bradycardia, 2. Asystole. 3. Hypotension 4.Peripheral Vasodilation. 5. Metallic taste. 6. Local necrosis. 7. Coronary and cerebral artery spasm. 8. Nausea and Vomiting. |
NOTES ON ADMINISTRATION | Pregnancy safety Category C. Do not use routinely in cardiac arrest. |
INCOMPATABILITIES/DRUG INTERACTIONS | May worsen arrhythmias secondary to digitalis toxicity. May antagonize the effects of verapamil. Do not mix with or infuse immediately before or after sodium bicarbonate without intervening flush. |
DOSE/ROUTE Adult | 500-1000mg (5-10 ml of a 10% solution) IV/IO push for Hyperkalemia and calcium channel blocker overdose. May be repeated as needed. DOSE/ROUTE Pediatric |
ONSET OF ACTION | 5-15 minutes |
PEAK EFFECTS | 3-5 minutes. |
DURATION OF ACTION | 15-30 minutes, but may persist for 4 hours (dose dependent). |
