MECHANISM OF ACTION  Increases cardiac contractile state (positive inotropic effect), May enhance ventricular automaticity  
INDICATIONS  1. Hypoglycemia, 2. Hyperkalemia 3. Magnesium sulfate overdose 4. Calcium channel blocker overdose 5. Adjunctive therapy in treatment of insect bites and stings  
CONTRAINDICATIONS  1. Hypercalcemia 2. V-fib 3. Digitalis toxicity  
ADVERSE REACTIONS  1. Bradycardia, 2. Asystole. 3. Hypotension 4.Peripheral Vasodilation. 5. Metallic taste. 6. Local necrosis. 7. Coronary and cerebral artery spasm. 8. Nausea and Vomiting.  
NOTES ON ADMINISTRATION  Pregnancy safety Category C. Do not use routinely in cardiac arrest.  
INCOMPATABILITIES/DRUG INTERACTIONS  May worsen arrhythmias secondary to digitalis toxicity. May antagonize the effects of verapamil. Do not mix with or infuse immediately before or after sodium bicarbonate without intervening flush.  
DOSE/ROUTE Adult   500-1000mg (5-10 ml of a 10% solution) IV/IO push for Hyperkalemia and calcium channel blocker overdose. May be repeated as needed. DOSE/ROUTE Pediatric  
ONSET OF ACTION  5-15 minutes  
PEAK EFFECTS  3-5 minutes.  
DURATION OF ACTION  15-30 minutes, but may persist for 4 hours (dose dependent).
