Define NURSING PROCESS: An organizational framework that links the PROCESS of THINKING with ACTIONS in nursing practice
What are the steps of the nursing process 1. ASSESSMENT 2. NURSING DX 3. PLAN OF CARE 4.IMPLEMENTATION 5. EVALUATION
What is subjective data information provided verbally by the patient
What is objective data factual information obtained through assessment and tests. What you can observe throught the 5 senses.
Documentation of data consists of 1. once collected, it is placed in patients medical record. 2. ID of significant problem is reported immediately to the RN OR MD 3. Info should be accurate and concise
