John Smith 75 year old male comes in for an assessment. When you examine his bloodwork, his GFR is less than 35. Is this normal or abdnormal? Abnormal
What other questions would you want to know about this patient? Past medical history, history of presenting illness, system assessment
John Smith has had hypertension (HTN) for 30 years and had been non-compliant with his medication regime for years. Is hypertension a precursor to kidney injury (CKD)? yes
How does HTN affect the kidneys? By constricting and narrowing the vessels in the kidneys, reducing blood flow.
How does congestive heart failure affect the kidneys? Reduced blood supply, increased fluid build up
Is it important to weight a patient with kidney failure daily? Yes
Why is it important to weight a patient with kidney failure daily? Because of the risk of fluid overload.
What important electrolyte can become elevated in someone who has kidney disease? potassium
What are some common interventions used with kidney disease? Dialysis, diuretics, fluid restriction, anti-hypertensives
If someone has a dialysis shunt (A/V Fistula) is it ok to perform a blood pressure, blood glucose or venipuncture on that arm? No-the fistula is the patient's lifeline. If it becomes damaged or becomes infected it can impact that person's life.
