Pharynx location Base of skull -> lower border of cricoid cartilage
Outer layer of pharyngeal muscles, innervation, function Superior/middle/inferior constrictors -> CN X -> circular muscles -> pharyngeal constriction to aid swallowing
Superior constrictor attachments Outer pharyngeal muscle -> pterygomandibular raphe (ligament) -> pharyngeal raphe
Middle constrictor attachments Outer pharyngeal muscle -> stylohyoid ligament -> pharyngeal raphe
Inferior constrictor attachments Outer pharyngeal muscle -> thyroid/cricoid cartilages -> pharyngeal raphe -> constriction as upper oesophageal sphincter
Inner layer of pharyngeal muscles, function Stylopharyngeus, palatopharyngeus, salpingopharyngeus -> longitudinal muscles -> elevate pharynx/larynx when swallowing
Inner pharyngeal muscle innervation CN X xcp stylopharyngeus (branchiomotor -> IX)
Stylopharyngeus attachments Inner pharyngeal muscle -> styloid process -> pharyngeal wall
Palatopharyngeus attachments Inner pharyngeal muscle -> palatine aponeurosis (hard palate) -> pharyngeal wall
Salpingopharyngeus attachments Inner pharyngeal muscle -> medial cartilaginous Eustachian tube -> pharyngeal wall
Pharyngeal plexus CN IX pharyngeal branch (stylopharyngeus), CN X pharyngeal branch, CN X superior laryngeal nerve external laryngeal branch, CN XI (cranial root gives some motor fibres to CN X), convey parasympathetic preganglionic fibres for pharyngeal glands
CN IX pharyngeal branch course Passes through oropharyngeal triangle -> inferior to palatine tonsil -> afferent limb of gag reflex
Oropharyngeal triangle borders Superior = inferior border of superior constrictor, anterior = posterior border of mylohyoid, posterior = anterior border of middle constrictor
Oropharyngeal triangle contents Hyoglossus/styloglossus muscles, lingual artery/vein, CN IX pharyngeal branch/XII, submandibular gland
Nasopharynx sensory innervation Maxillary nerve CN Vb -> passes through sphenoid palatovaginal canal
Oropharynx sensory innervation CN IX
Laryngopharynx sensory innervation CN X
Waldeyer’s ring 4 groups of 6 specialised lymphatic tissue at entrance of digestive/respiratory tract -> 1 adenoid/nasopharyngeal, 2 tubal tonsils, 2 palatine tonsils, 1 lingual tonsil
Adenoid tonsil Unpaired -> roof of nasopharyngeal cavity posterior to choanae -> enlargement will obstruct children nasopharynx
Tubal tonsil Paired -> medial to Eustachian tube opening -> removed in persistent tonsilitis -> enlargement will obstruct Eustachian tube and lead to middle ear secretion accumulation
Palatine tonsil Paired -> btwn palatoglossal (ant)/palatopharyngeal (post) arches -> largest tonsils -> in tonsilectomy, facial artery/ECA/CN IX endangered
Lingual tonsil Unpaired -> post 1/3 of dorsal tongue -> inflammation will lead to painful swelling
