What is the desired effect of testosterone therapy for a patient with "low t" syndrome? Spermatogenesis, libido, and weight gain.
What effects does testosterone have on women? It will cause masculinization, Hair growth/breast atrophy/deepening of voice/suppression of menstruation
What are some important blood tests to have done when taking testosterone therapy. ? PSA (Can accelerate prostate CA) Check lipids (it can cause dyslipidemia and accelerated ASHD) Need to have a baseline testosterone level and this needs to be checked every 6 months.
Testosterone Transdermal patches...... QD/Androderm/Rotate sites/apply to upper body/REMOVE BEFORE MRI!!!
Testosterone topical gels......... Apply QD/Androgel/Testim/😲BLACK BOX WARNING!!! caution for children-even with dried gel! Skin-to-skin transfer is possible!! Apply to upper chest and arms.
Injectable testosterone ...... IM every 2-4 weeks/Depo-testosterone)/Very common/Potential for CIII diversion
Buccal tablet for testosterone........ Q 12 HRS (Striant)/Do not chew or swallow/BID/$$/Tablet doesn't dissolve/Not used very much
PO Testosterone ....... BID-QID oxandrolone/used for weight gain in emaciated patient/rarely used/😲BLACK BOX WARNING this med can cause hepatic tumors/cancers!!! /Usually use megestrol instead
Testosterone use remains common in men with CAD despite the risks. True or false True Off label use remains high in men with CAD compared to those without/CAD and testosterone mixed increased risk of heart attack SIGNIFICANTLY!!!!
What is the mechanism of action for anabolic inhibitors? (Finasteride/Dutasteride) Antagonist/Reduce the amount of testosterone from being able to bind to its receptors.
What is BPH? Benign prostatic hypertrophy/Treated with anabolic inhibitors (Finisteride)
What are some ADR'S (adverse drug reactions) for the anabolic inhibitors? (finasteride/dutasteride) Decreased libido/Impotency
What are some conditions treated with anabolic inhibitors (finasteride/dutasteride) ? BPH/Cancers of male reproductive organs/Male-pattern baldness
What can happen if a pregnant woman absorbs an anabolic inhibitor? (finasteride/dutasteride) It can cause abnormalities in fetal development of a male child if a pregnant woman absorbs the drug/Caregivers that are pregnant should NEVER TOUCH CRUSHED tablet or come into contact with the semen of a patient using this drug!😉
What is the primary treatment of BPH? (Only relieves symptoms) hyperplasia of prostate with urinary hesitancy & incontinence Primarily ALPHA BLOCKERS. (OSIN FAMILY) Relaxes the bladder neck & prostatic smooth muscle to ease urine restriction. THIS DOES NOT SLOW DOWN THE GROWTH OF THE GLAND! (DOES NOT SHRINK PROSTATE!)
What is something you really need to monitor on a patient taking an alpha blocker? HYPOTENSION- need to warn about orthostatic hypotension/change positions slowly (first dose packs a big punch)
** Newer agents are more selective for bladder/prostate alpha receptors than others & therefore less hypotension risk name two newer drugs that fall under this. tamsulosin (Flomax)/alfuzosin (Uroxatral)
What is the 2nd line choice of drugs after Alpha blockers for BPH 2nd line choices: Anabolic Inhibitors. to block conversion of testosterone to an androgenic prostate hormone thus reducing size of gland.
What is Erectile Dysfunction? Inability to obtain and/or sustain erection block phosphodiesterase, leading to vasodilation and increased blood flow to penis. Need to allow 30 mins. for effect; 1-2 hours if taken with high-fat meal.
What is an adverse side effect with ED meds? possibly severe hypotension if concurrent use with nitrates & alpha blocker (i.e. Angina or BPH) can cause dizziness with postural hypotension. Can be fatal.
What drugs are for the treatment of ED? Drugs: the “afil” family/sildenafil (Viagra®)/vardenafil (Levitra®) QD prn/tadalafil (Cialis®) which is short and long acting 2.5- 5mg QD to10 - 20mg q3d prn
What are Treatment options for Male Pattern Baldness/Alopecia? PO - testosterone inhibitors such as finasteride (Propecia®)/ Topical - antihypertensive minoxidil (Rogaine® )
