Chief Complaint Chap2 The main reason for a visit
Differential Diagnosis A list of possibilities for the diagnosis
Family history Any significant illnesses that run in the patient's family
History of present illness The story of the patient's problem
Past medical history Other significant pass illnesses
Social history A record of habits like smoking, drinking, drug abuse, and sexual practices that can impact health
Clinic note Written by medical professional and documents an office visit
Consult note Written by a physician, usually a specialist, and provides an expert opinion on a more challenging problem
Discharge summary Written by a medical professional in a hospital, describes when and why the patient was admitted, documents a longer stay, and starts with the assessment.
Operative report Documents a surgery in detail
Pathology report Provides reasons for a test, what was seen on the test, and as assessment
Prescription Provides directions for a medication
Radiology report Explains reasons for image, how image was performed, what was seen on image, radiologist's assessment, and sometimes a recommendation
Lethargic A decrease in level of consciousness; in a medical record, this is generally an indication that the patient is really sick
Oriented Being aware of who he or she is, where he or she is, and the current time
