Abbreviations used for symptoms/ exam findings

SOB | shortness of breath |
NAD | no acute distress (patient does not display intense symptoms) |
A&O | alert and oriented (patient is aware of what is going on) |
CTA | clear to auscultation (description of normal sounding lungs) |
WNL | within normal limits |
WDWN | well developed, well nourished (patient is growing/ has grown appropriately, does not appear malnourished) |
RRR | regular rate and rhythm (description of normal heart) |
CV | cardiovascular |
HEENT | head, eyes, ears, nose and throat |
PERRLA | pupils are equal, round, and reactive to light and accommodation |
