Chapter 9

Airway Management

QuestionAnswer
airway the passageway by which air enters and leaves the body. The structures of the airway are the nose, mouth, pharynx, larynx, trachea, bronchi, and lungs
bronchoconstriction the contraction of smooth muscle that lines the bronchial passages that results in a decreased internal diameter of the airway and increased resistance to airflow
gag reflex vomiting or retching that results when something is placed in the back of the pharynx. this is tied to the swallow reflex
head-tilt, chin-lift maneuver a means of correcting blockage of the airway by the tongue by tilting the head back and lifting the chin. used when no trauma, or injury, is suspected
jaw-thrust maneuver a means of correcting blockage of the airway by moving the jaw forward without tilting the head or neck. used when trauma, or injury, is suspected to open the airway without causing further injury to the spinal cord in the neck
nasopharyngeal airway a flexible breathing tube inserted through the patient's nostril into the pharynx to help maintain an open airway
oropharyngeal airway a curved device inserted through the patient's mouth into the pharynx to help maintain an open airway
patent airway an airway that is open and clear and will remain open and clear without interference to the passage of air into and out of the body
stridor a high-pitched sound generated from partially obstructed airflow in the upper airway
suctioning use of a vacuum device to remove blood, vomitus, and other secretions or foreign materials from the airway
cricoid cartilage cartilage that forms to protect the lower aspect of the larynx and the superior aspect of the trachea; top ring of cartilage protecting the trachea; surrounds 360 degrees around the trachea
carina the location that the trachea branches into 2 stems of bronchi
paranasal sinuses 4 paired air filled spaces that surround the nasal cavity
pharyngeal tonsil a collection of lymphoid tissue within the mucosa of the roof of the nasopharynx
eustachian tube a canal that connects the middle ear to the nasopharynx
hyoid bone a horseshoe-shaped bone situated in the anterior midline of the neck between the chin and the thyroid cartilage
palatine tonsil bundles of lymphatic tissue located in the lateral oropharynx
hard palate the front, bony part of the roof of the mouth
soft palate the muscular part at the back of the roof of the mouth
hypopharynx bottom part of the pharynx
What is the purpose of the laryngopharynx? to provide structure to and protect the opening to the trachea
How many rings of cartilage protect the trachea? 16
What are the common obstructions to the airway? toys, food, blood, vomit, tongue, burns, blunt-force trauma, infection,
What are the top questions to consider when assessing a patient's airway? is the airway open? will the airway stay open?
What is an immediate indicator that a patient is able to adequately move air? speaking
What are the steps of airway assessment? is the airway open? is the patient speaking? look. listen. feel. will the airway stay open? are there potential threats that may develop later?
What are the signs of an inadequate airway? no signs of breathing, evidence of foreign bodies in the airway, no air can be felt or heard, patient is unable to speak or has hoarse voice, chest movements are absent, abdominal breathing, breath sounds are diminished, noises such as stridor or wheezing
What is the most effective way to clear the patients airway? with a wide bore rigid tip Yankauer suction device
How do you measure an OPA? from the corner of the mouth to the tip of the earlobe
What are the steps of inserting an OPA? place patient on the back, perform a crossed finger technique, point the airway device toward the roof of the mouth, slide the device along the roof of the mouth until you hit the soft palate, rotate until it points to the pharynx, monitor the patient
What are the steps of inserting a NPA? measure the NPA, lubricate the outside, push the tip of the nose upward, insert airway into the nostril
What are the necessary attachments? tubing, suction tips, suction catheters, collection container, container of clean or sterile water
