HIT Chapter 7
Indexes, Registers, and Health Data Collection
Question | Answer |
---|---|
Admission/discharge/transfer system | used to input patient registration information, which results in creation of an automated master patient index database that allows for the storage and retrieval of information |
data comprehensiveness | all required data elements are present in the patient record |
data consistency | ensures reliability of data regardless of the way in which data are stored, displayed, or processed. |
data dictionary | a collection of data element definitions |
data definition | data elements should have defined meanings and values so all present and future users understand the data. Facilities create a data dictionary to facilitate this process. |
data set | establishing of a standard method for collecting and reporting individual data elements so data can be easily compared. |
data reliability | data is reliable if it is consistent throughout all systems in which it is stored, processed, and/or retrieved. |
data precision | yields accurate data collection by defining expected data values. |
case report forms | submitted by health care facilities and providers to report data to sponsoring agencies, facilities and organizations. |
comparative data | category of health care data used for health service outcomes measurements and research. |
daily census count | number of inpatients present at census taking time. Usually taken at midnight. |
data accuracy | data that is error free and correct. |
data analysis | process of translating data into information utilized for an application. |
data collection | process by which data elements are accumulated. |
data application | purpose for which data is collected |
daily inpatient census | official count of inpatients present at midnight and is calculated each day. |
line diagram | displays data over a period of time, also called run chart |
pie chart | displays component parts of data as it relates to the whole |
register | in health care, a record of admissions, discharges, birth, deaths, operations, and other events |
procedure index | organized according to ICD-9-CM and/or CPT/HCPCS procedure/ service code |
magnetic stripe card | contains an electromagnetic surface capable of holding a small amount of information. |
vital statistics | complied for events, including births, deaths, fetal deaths, marriages, and divorces |
relational database | contains a limited two dimensional structure that does not allow for complete trend analysis. |
index | serves to guide, point out or otherwise facilitate reference an alphabetized list of names |
admission register | maintained by admissions office, and is organized by admission date; contents include patient's name, patient number, admitting physician, admission date, admission diagnosis, and room number. |
aggregate data | category of health care data based on performance and utilization and resource management. |
arithmetic mean | mathematical average |
automated case abstracting systems | allows health care facilities to calculate PPS reimbursement, generate case abstract statistics, review patient charges concurrently... |
automated MPI | resides on a computer and consists of a database of identification data about patients who have received health care services from a facility. |
average daily census | average number of inpatients treated during a given time period; calculated for varying time periods. |
average length of stay | total length of stay divided by the number of patients discharged. |
data granularity | each attribute and value of data is defined at the correct level of detail. |
