Tag Out Users Manual

QuestionAnswer
Who will be notified if any discrepancy and/or conflict is noted with a posted tag? Authorizing officer and the RA Representative
Where will the Authorizing officer document discrepancies/conflicts and the resolution? in their logs
What immediate action shall be taken if there is a missing or damaged tag? appropriate preventative measures to preclude changes to the item until it can be re-tagged.
When will a verification check be performed? if the position/condition of a valve/switch is in doubt
Who is responsible for taking action or recommending further action be taken based on their evaluation of the circumstances surrounding a missing or damaged tag? authorizing officer or RA representative
What will be verified by the Authorizing officer and RA representative, to determine any effects that may have resulted from changes to the item while a tag was missing/damaged? plant conditions/system status
If a tag is missing what must be done to ensure continued protection of personnel and equipment until a replacement tag is posted? stop affected work
Once a missing/damaged tag is replaced what number will it be assigned and documented in the TORS? the same number as the original tag
What will be indicated in the TORS in block 19 for replacement tags? missing/damage - replaced
When may a tag may be replaced and relocated from a breaker cover to a breaker? if positive controls are in place
Who may authorize work on any component that has a danger tag or caution tag? Department Head
What is not allowed under any circumstances while working on a tagged component? work that would be likely to affect component position or position indication, or result in breaching the isolation boundary
