DAW 0 | NO DISPENSE AS WRITTEN (Substitution Allowed) (or no product selection indicated) • Use the DAW 0 code when dispensing a generic drug; that is, when no party, requests the branded version of a multi-source product. |
DAW 1 | PHYSICIAN writes DISPENSE AS WRITTEN • Use when the Prescribing Provider specifies the branded version of a drug on the hard copy prescription or in the orally communicated instructions. |
DAW 2 | PATIENT REQUESTED • Use this code when the Participant requested the branded drug even though the original prescription did not indicate "Dispense As Written". |
DAW 3 | PHARMACIST SELECTED BRAND |
DAW 4 | GENERIC NOT IN STOCK |
DAW 5 | BRAND DISPENSED, PRICED AS GENERIC • Use when dispensing a brand as a generic. • Claims submitted with DAW 5 will be reimbursed at the generic price |
