Salivary Tumors

Salivary Gland Tumors

QuestionAnswer
Major Salivary Glands = ? * Parotid, Submandibular, and Sublingual
Minor = ? * scattered 500-700 in upper aerodigestive tract
The Parotid Gland basics ? * largest gland..... * Facial nerve divides the two lobes, superficial (80%) and deep (20%)..... * Composed almost entirely of serous acinar cells with striated ducts ...... * superficial lobe = most common neoplasm place
Submandibular gland basics ? * Composed of both mucous and serous secretory cells, so see dark and light staining cells....... * 2nd largest gland
Sublingual Gland basics ? *smallest, and produces only mucous..... *
Salivary Gland Tumor Basics ? * Happen more often in the Parotid Gland (benign usu.).... * Sublingual tumors are the least, but when there is one there, it is almost always Malignant
Benign Tumors = ? * Pleomorphic Adenoma -- Warthin Tumor
Malignant Tumors = ? * Mucoepidermoid Carcinoma -- Adenocarcinoma (NOS) -- Acinic Cell Carcinoma
Pleomorphic Adenoma basics ? * benign MIXED tumor, usu in Paortid gland.... * exhibits both epithelial (ductal and nonductal) and mesenchymal (chondroid myxoid and osseous)
Pleomorphic Adenoma Histo? * See well-demarcated tumor with adjacent normal salivary gland parenchyma and also see epithelial cells and myoepithelial cells within a chondroid matrix bc it is mixed...... *
Pleomorphic Adenoma CxSx ? * Painless, slow grower, and mobile mass ..... * Can reoccur if you miss a piece of it.... * The longer it is there, the higher chance it turns Malignant --> Carcinoma ex pleomorphic adenoma , which is aggressive
Warthin Tumor (Papillary Cystadenoma Lymphomatosum) basics ? * usu. in Parotid Gland, Older Men, and getting one increases with smoking...... * arise from inclusion of salivary tissue and lymphoid tissue
Warthin Tumor Histo ? * Gross - see we cyctic, well encapsulated mass..... * Histo: see epi. with lymphoid elements.... * Double layer of lining cells:; surface palisade of columnar cells with abundant, finely granular, Eo cytoplasm
Mucoepidermoid Carcinoma basics ? * Most common form of primary malignant tumor of the salivary glands..... * T(11;19)(q21;p13) creates a fusion gene of MECT1 and MAML2 genes, to get high cellular prolif.
Mucoepidermoid Carcinoma Cx Features ? * can grow very large (8cm), no capsule, and if high grade tumor = higher recurrence rate and higher mortality rates
Mucoepidermoid carcinoma Histo ? * Mucicarmine stains the mucin pink-red ...... * Cords, sheets or cystic configurations Of squamous, mucus or intermediate cells The intermediate cell ectypes have squamous features with mucus filled vacuoles
Adenoid Cystic Carcinoma basics ? * very uncommon and rare tumor, that invades nerves = CAUSE PAIN....* slow grower, and has HIGH RECURRENCE rates.... * Can Mets after removal yrs after.... * Very LOW survival rates
Adenoid Cystic Carcinoma Histo ? *see lots of glands, cribriform pattern, Spaces between the cells
Acinic Cell Carcinoma basics ? * Composed of cells resembling normal serous acinar cells of salivary glands, and arise mostly in Parotid gland..... * Very RARE to reoccur if resected.... * HIGH survival rate also
