Asthma Drugs
USMLE Step 1
Drug | MOA & ADRs |
---|---|
Metaproterenol | MOA: Beta-2 agonist used for mild, intermittent asthma, rapid onset & provide relief for 4-6 hrs, potent bronchodilators, not destroyed by COMT; ADRs: little beta-1 effect, but can cause cardiac stimulation resulting in heart palpitations |
Terbutaline | MOA: Beta-2 agonist used for mild, intermittent asthma, rapid onset & provide relief for 4-6 hrs, potent bronchodilators, not destroyed by COMT; ADRs: little beta-1 effect, but can cause cardiac stimulation resulting in heart palpitations |
Albuterol | MOA: Beta-2 agonist used for mild, intermittent asthma, rapid onset & provide relief for 4-6 hrs, potent bronchodilators, not destroyed by COMT; ADRs: little beta-1 effect, but can cause cardiac stimulation resulting in heart palpitations |
Cromolyn | MOA: used for prophylaxis of moderate asthma that is a mast cell stabilizer & NOT useful for acute attacks, ADRs: bitter taste, as well as irritation of the larynx & pharynx |
Beclomethazone | MOA: Corticosteroid for severe, chronic asthma serves to decrease activation of macrophages, eosinophils & T cells, reverse mucosal edema, decrease capillary permeability & - release of leukotrienes; *Note 80% PO metabolized by liver, spacer used* |
Triamcinolone | MOA: Corticosteroid for severe, chronic asthma serves to decrease activation of macrophages, eosinophils & T cells, reverse mucosal edema, decrease capillary permeability & - release of leukotrienes; *Note 80% PO metabolized by liver, spacer used* |
Flunisolide | MOA: Corticosteroid for severe, chronic asthma serves to decrease activation of macrophages, eosinophils & T cells, reverse mucosal edema, decrease capillary permeability & - release of leukotrienes; *Note 80% PO metabolized by liver, spacer used* |
Theophylline | MOA: Phosphodiesterase - that increases the amt of Ca, it is used for more severe asthma; ADRs: not freq used b/c many interactions w/ wide range of drugs, thus has a low therapeutic index, causes seizures and/or fatal arrhythmias |
Zileuton | MOA: leukotriene - (direct inhibition of lipoxygenase) decrease bronchoconstriction txing the sx, thus not 1st-line drugs in tx of asthma |
Zafirlukast | MOA: leukotriene - (leukotriene receptor blocker) decrease bronchoconstriction txing the sx, thus not 1st-line drugs in tx of asthma |
Montelukast | MOA: leukotriene - (leukotriene receptor blocker) decrease bronchoconstriction txing the sx, thus not 1st-line drugs in tx of asthma |
Methylprednisone | MOA: IV used for 'Status asthmaticus' a severe, unrelenting exacerbation of asthma |
Prednisone | MOA: PO used for 'Status asthmaticus' a severe, unrelenting exacerbation of asthma |
Epinephrine | MOA: acute anaphylactic shock (Type 1 Hypersensitivity) |
