What is the importance of the formulation record?   The formulation record provides a constant source of documentation for preparing the formulation. It is a recipe book for compounding formulas.

What information is contained on the compounding record?   It list the name, strenght, dosage form of the preparation, ingredients and the quantities, calculations regarding dilutions, equipment needed for preparation, and the mixing instuctions.

Define balance and identify which balance is required in every pharmacy?   A balance is an instrament used for determining the weight of a substance. Class A prescription balance is required.

Identify the techniques used in weighing ingredients.   Gather and organize materials, level the balance on a flat level surface that is free from drafts and breezes, place weighing boats on pans, check balance and beam, adjust the weighing aparatus for the right pan, and place the material in the left pan.

What is the difference between a conical and cylindrical graduate?   Conical graduates have a wide mouth and a wide base. Cylindrical graduates provide greater accuracy in measurment.

Identify the procedure used in measuring a liquid.   Remove cap from stock bottle, hold the graduate in the left hand and stock bottle in the right, raise and hold the graduate so that the left graduation point to be raise at eye level, pour liquid into graduate to the desired measurement, replace cap.

Why would a patient need an extemporaneous compound?   Because they are allergic to a certain ingredient in the compound so they need a formula to be suited and fit to their needs.
