cardiac glycosides: mechanism of action (digoxin & digitoxin) -slows down the heart to allow it to fill up more and then has a stronger pump
phosphodiesterase inhibitors 2 agents: inamrinone & milrinone -both have adverse effects of hypotension -used for short term management of HF (given when pt. has not responded to treatment with digoxin, diuretics, or vasodilators
positive inotropic agents: nursing implications assess weight and potassium levels -weigh them 1st thing every morning wearing basically the same thing b4 they eat or drink. weight is the best indicator of fluid -b4 giving any dose count apical pulse for 1 full min
positive inotropic agents: nursing implications -avoid giving digoxin with high-fiber foods (fiber binds with digitalis) -pt's should report weight gain of 2 more more pounds in 1 day or 5 or more pounds in 1 wk (if they are gaining that much weight it is b.c of fluid and it should be reported)
digoxin serum level 0.5-2ng/mL
when teaching a pt. about the S/S of cardiac glycoside toxicity, the nurse should alert the pt. to watch for flickering lights or halos around lights
during assessment of a pt. who is receiving digoxin, the nurse monitors for findings that would indicate an increased possibility of toxicity, such as serum potassium level of 2.0 mEq/L
