Home Health Care Visits/Reimbursement Must be authorized by physician. Plan of care: diagnosis, mental status , types of services needed, supplies & equip ordered, freq. of visits, prognosis, rehab potential, funtional limitations, nutrition, meds and treatment
Disability Levels Level I Slight limitation in one or more ADL; able to work
Disability Levels Level II Moderate limitation in one or more ADL; able to work but workplace may need modifications.
