What is substance abuse? Over indulgance of a chemical substance resulting in dependance that interferes with life sctivities
Substance abuse orders are usually the result of interpersonal inadequacy
The substance abuser believes that the substace will help them reduce strss or assist with coping skills
Does abuse of a substance require addiction? no
stress that are situatioan are external stressors
stressers that are caused by disease or withdrawl of a drug internal stressors
the center for emotional responses controls the involuntary nervous system with organ fx and biorhythms hypothalmus
biologic rythms that the hypothalmus controls sex drive internal drive ,perception of fear & rage
sets the stage for both physical & psychological addiction cyclic ingestion
assesment for substance abuse includes prior hx for substance abuse, s/s of abuse, s/s of withdrawl, d/c of pt to proper tx facility
stimulants are prescribed for ADHD obesity narcolepsy
stimulants increase mental alertness, sense of well being, capisity to work, notor skills, metablolism
how do stimulants increase metabolism by increaseing respiratory & cardiac fx
stimulants include amphetamines, methamphetamines, cocaine
s/e of amphetamine abuse agression energy, insomnia, mental issues
amphetamines counteract barbituarates
medications used for the pt going through with drawl from amphetamines thorazine, valium
methamphetamines produce high levels of dopamine
methamphetamines are neurotoxic to cells that contain seritonin
results of meth use hyperthermia, convulsions, CVA, brain attacks, cardiovascular collapse than death
uses/effects of cocaine anesthetic, appitite suppresent, stimulant, vasoconstrictor to stop bleeding, gives euphoric feeling
cocaine interferes w/reabsorption of dopamine (if dopamine is depeleted than effects are opposite)
depletion of dopamine basis for cocaine cravings
after cole use stops what dopamine agonist is used Pariodel
A coke user with s/s similar to ETOH withdrawl, sweating dialated pupils, psychomotor agitation, increased BP, increased heartrate low level of coke intoxication
high fever agitation, arrhythmia seziuresm, paranoid schizophrenic syndrome w/hallucinations high level of coke intoxication
hallucinations from coke use coke bugs
used when pt is in a psychotic episode from coke haldol
detox from coke depends on symptoms
detox meds for coke use Valium-anxiety;Phenobarbs-seziures, Tofranil-depression, Inderal - Increased BP & tachycardia
Hallucinogens a chemical that produces mind altering or thrill seeking efects
common hallucinogens PCP Peyote, MDMA, LSD, MJ
Peyote used in Native American religious rituals found in mushrooms
Depakote may potentiate effects of PCP
physical symptoms of hallucinaogens dialted pupils, tachycardia, sweating, palpitations, blurred VA
physiologic symptoms of hallucinogens includes depersonalization paranoid ideations anxiety body image changes intence perceptions
person on hallucinogens is at risck for danger to self & others because of unpredictable behavior
valium used for tx of muscle spasms anxiety seziures
haldol tx of psychotic behaviour
CCB tx to prevent vasospasms & to decrease hallucinations
good adjunct for sleep, decreases activity in brain, sedates, releases inhibitions, decreases motor activity can cause resp depression depressants
alcohol Benzos Barbturate types of depressants
alcohol abuse drinking reguardless of consequences
phychological effects of chronic alcohol abuse depression loss of self respec malnutrition, alination, inefections
rate at which 95% of alcohol is metabolized in the liver 10mil per 90 min
legal def of drunk Blood Alcohol Level > 0.10%
a known alcohol abuser with confusion disorintation, and no recolection of anything familiar may be in (blank)
a known alcoholic experinceing confusion, disorientation, no recolection of anything familliar may be in DT's
Pt gogin through alcohol withdrawl should maintain fluid intake of 3000 ml per day
alcohol inhibits the absorption of vit b12
pt on anabuse should avoid anything with ETOH in it
unconsciousness passing out
anterograed amnesia or loss of short term memory w/retention of remote memories blacking out
damage to hipocampus & surrounding tissue r/t heavy alcohol ingestion Korsakoff's (w/o tx is irreversable & has severe impairment)
neurological disease characterized by ataxiaconfusion & 6th cranial nerve palsy it follows acute episode of alcohol encephalopathy wernicke's Encephalopathy
Wenriche's Encephalopathy responds well to large doses of parental thiamine in acute early stages
FAS physical effects growth defects, heart defects, malformed facial features, low birth weight
mental deficits r/t FAS mental retardation, learning problems hyperactivity
Barbituates, seditives hypnotics(xanax, valium, ativa,, ambian0 over prescribed for stress & sleep disorders & have high risk for abuse
are partialy metabolized in the liver & unmetablolized parts before active metabolites & stored in the fatty tissues Barbiturates/Seditives
Narcotics made from asian poppy or produced as synthetic drug effects of opium
analgesic & euphoric effects of narcotics
opoids Heroin, morphine, Oxycotin, demerol, codine, methadone
symptoms of narcotic overdose constricted pupils, euphoria, psychomotor retardation slurred speach drowsiness
s/s in infant born to mom addicted opoids tremors, irritable, high pitched cry increased resp fever
tx of choice for opoid withdrawl methadone-blocks cravings
psychoadictive stimulant nicotime
nicotine causes vasoconstricton & increases peripheral resistance
anabolic steroids (muscle building) & androgenic (increase masculinity) steroids are ilegal
extra hair on woman, agression s/e of steroids
complicates dx & treatment of substance abuse because of synergistic paradoxial additive & potentiating effects polydrug use
substance induced disorders intoxication withdrawl cognitive d/o mood d/o etc
maladaptive pattern of substance use with clinical significant impairment is distress substance dependance
needing larger amount than what is intended can reduce or regulate substance devotin great deal of time to obtain substance tolerance
substance abuse characterized by pattern of repeted use of substance that is maladaptive in that significant adverce consequences occur such as faliure to fullfill major role obligations
substance intoxication reversable syndrome of maladaptive physiologiv & behavior that are due to the effects of a substance on a persons CNS
theory that tries to determine a genetic predisposition to alcoholism uses pharmocologic management instead of psychosocial biogenetic biophysical
substance abuser is viewed as regressed & fixated at pregenital oral level of psych developmetn psychological theory
abuser describes learning to drink by watching a family membe thinks it may remedy pain social cultural theroy
person who alternately recues & blames the addict codependant
enablers takes the roles of protector controller & blamer but isnt effective in alltering the diease
1 parent is the addict 1 parent is the codependant /enabler, kid is the defensive personality disfunctional family roles
alcoholics are at high risk for suicide assess for selfdestrucitve behavior & events that represent loss
