osteoarthritis (degenerative joint disease) noninflammatory, localized, progressive disorder involving deterioration of articular catilages + subchondral bone + formation of new bone -affects hands, knees, hips, + lumbar +cervical ligaments of spine -become stiff, swell, bony protuberances, pain
ankylosing spondylitis chronci progressive inflammation of spine, sacroliliac, + larger joints of extremities, leading to bony akyosis + deformity common in men by 10:1
osteoporosis decrease in skeletal bone mass occuring wen rate of ben resorption is > then bone formation -postmenopausal white women
atrophy loss of muslce mass is exhibited as lack of fullness surrounding deltoid muscle
dislocated shoulder exhibited wen hunching shoulder forward + the tip of clavicle dislocates -occur w/trauma involving abduction, extension, + rotation
joint effusion swelling from excess fluid in joint capsule, from rheumatoid arthritis
tear of rotator cuff haunched position + limited abduction of arm -(+) drop arm test: if arm is pasively abducted @ shoulder cant hold arm and drop @ side
frozen shoulder-adhesive capsulitis fibrous tissues form from joint capsule, causing siffness, progressive limitated of mothion + pain -associated with prolonged bed rest
subacromial bursitis inflammation + swelling of subacromial bursa over shoulder cause limited ROM +pain with motion
olecranon bursitis large soft knob or goose egg, + redness from inflammation of olecranon bursa -localized + easy to c bc burse lies just under skin
gouty arthritis joint effussion or synovial thickening, seen first as bulge or fullness in grove -redness + heat can extend beyoned are -soft, boggy, or fluctuant fullness to palpation
subcutaneous nodules raised, firm, nontender nodule that occur w/RA -common site in olecron burse -skin sliddes freely over nodule
epicondylitis-tennis elbow chronic disabling pain @ lateral elbow humerus, radiatesdown to forearm
ganglion cyst round, cystic, nontender noduel overlying a tendon sheath or joint capsule
carpal tunnel syndrome w/atrophy of thenar eminence caused by chronic repetive motion (30-60 women) -symptoms of CTS include pain, buring + numbness, + Phalens + Tinels test
ankylosis wrist in extreme flexion due to sever RA
Colles' fracture nonarticular fracture of distal radius w or w/o fracute of ulna -wrist looks puffy, with silver fork deformity
dupuytren's contracture chronic hyperplasia of palmar fascia causes flexion contractures of digits, 1st in 4th digit then in 5th , then3rd -men past 40, bilateral
swan-neck flexion contracture resemvles curve of swan neck -occurs with chronic RA + is often
boutonniere deformity knuckle looks as if it is being pushed through a buttonhole
ulnar deviation or drift fingers drift to ulnar side bc of stretching of articular capsule + muscle imbalance -caused by chronic RA
degenerative joint disease or osteoarthritsis characterized by hard, nontender nodules, 2 to 3 mm or more -osteophytes of distal interphalangel joins are called Heberdens nodes, proximal interphalangela joins are bouchards nodes
acute RA painful swelling of joins, with fusiform or spindle-shaped swelling of soft tissue of proximal interphalangeal joins -symmetric swelling, warmhands, veins engorged
syndactyly webbed fingers are congenital deformity, requires surgical seperation -metacarpasls + phalanges of webed fingers r different lengtsh + joints do not line up
polydactyly extra digit r congential deformiy, occur at 5th or thymb
mild synovitis loss of normal hollows on either side of patella, which r replaced by mild distention -occur w/synovial thickening or effussion
prepatellar bursitis localized swelling on anterior knne between patell + skin
swelling of menisci localized soft swelling from cyst in lateral meniscus hsows at midpoint of anterolateral joint line
osgood-schlatter disease painful swelling of tibial tubercle just below the knee, probably from repeated stres on patellar tendon -occur in puberty
chondromalacia patellae degeneration of articular surface of patellae -occur in female
achilles tenosynovitis itis of tendon sheath near ankle, produces a superficial linear swelling + localized tenderness
tophi with chronic gout hard, painless nodule over metatarsophalangel joint of first toe -tophi are collections of Na urate crystales due to chronic gout in + around joingt that casue extreme swelling + joint deformity
acute gout involves 1st metatarsophalegel joint -redness, swelling, heat + extreme tnederness
callus hypertrophy of epithelium develops bc of prolonged presure, commonly on plantar surface of the 1st metarsal head
plantar wart vascular papillomatous growth probably due to a virus + occurs on sole of foot, common at ball
hallux valgus with buninon + hammertoes 1. coomon deformity of RA, lateral or outward deviation of great toe w/medial prominence of head of 1st metarsal 2. buinon: inflammed bursa that forms at pressure point 3. hammertoe: deformity of 2,3,4,5 toes 4. corns: thickening of soft tissue
ingrown toenail misnomer; nail does not grow in but the soft tissue grows over the nail + obliterates the groove
scoliosis lateral curvature of thoracic + lumbar segements of spine -functional: apparent wen standing, flexible, -structual: fixed, stand + bend,
herniated nucleus pulposus nucleus ruptrues into spinal canal + puts presure on local spinal nerve root -sciatic pain, numbness, + paresthesia of involved dermatome; decreased mobilitiy, low tenderness
talipes equinovarus (clubfoot) congenital, rigid, + fixed malposition of foot: inversion, forefoot adduction, foot pointing downward
spina bifida incomplete closure of posterior part of vertebrae results in nerual tube defect
phalens test reproduce numbnees + burning in person with CTS (-) no symptoms
tinel's sign in CTS percussion of median nerve produce burning + tingling along its distribution
