cholecystitis chenodiol (Chenix)-->dissolution of stone cholestryamine (Questran)-->binds w bile salts promotes excretion. meperidine (Demerol) for pain antiemetics
Impetigo cantagiosa Burow's solution, topical antibiotics: mupirocin(Bactroban), bacitracin (Baciquent), polymixin B sulfate (neomycin)neosporin. PO antibiotics if severe: penicillin, erythromycin, dicloxacillin
Tinea Capitis Oral griseofulvin 6-8wks, selenium sulfide shampoo 3x q week/2wks, topical antifungals clotrimazole and miconazole
Tinea corporis po griseofulvin, topical antifungal clotimazole(Lotrimin) miconazole(Monostat,Micatin), tolnaftate(Tinactin)
Tinea pedis topical antifungal initially liquid or powder tolnaftate. If not effective po griseofulvin. Burows soaks-->glucocorticoid steriod cream
Tinea unguium oral antifungals terbinafine(Lamisil), itraconazole(Sporanox)
Tinea cruris "jock itch" Local application of tolnaftate liquid
Herpes Simplex Virus Type 1 acyclovir(Avirax,Zovirax) analgesics motrin, tylenol, topical anesthetics. Wet astringent compresses
Herpes Zoster oral analgesics/sedatives, antihistamines, acyclovir(Zovirax), corticosteriod systemic or inj@lesion. Burow's compresses.
Pediculosis humanus capitis permethrin 1%(Nix)-->nits and lice, pyrethrin (RID or A200), lindane(Kwell), malathion 0.5%(Ovide)
Scabies topical scabicide. permethrin 5%(Elimite), lindane 1%(Scabene), crotamiton 10%(Eurax), ivermectin. antipruritic lotions and antibiotics if sec infection.
Contact Dermatitis topical corticosteriods, oral corticosteriods, oral histamines, burows compress, calamine lotion, cornstarch, baking soda, oatmeal baths.
Seborrheic Dermatitis antiseborrheic shampoo(saleneum) Sebulex, Fostex Medication, P&S Commerical Baby Shampoo, mineral oil or baby oil-->crust removal. Tar shampoos for adult-->tgel, topical corticosteriods, antibiotic if infection.
Diaper dermatitis topical steriod cream/ointment, anti monilial nystatin and triamcinolone(Mycolog), clotrimazole(Lotrimin), clotrimazole and betamethasone(Lotrisone), clioquinol(Vioform), clioquinol and hydrocortisone(Vioform HC), oral nystatin(significant), antibiotics
Intertrigo topical corticosteriod, burow's compress, topical antifungal nystatin, ketoconazole(Nizoral), clotrimazole.
Eczema (AD) Antihistamines fexofenadine(Allegra) loratadinet(Claritin)day hydrooxyzine(Atarax) diphenhydramine(Benadryl), topical corticosteriods hydrocortisone, triamcinolone(kenalog, Aristocort), betamethasone(Valisone). Antibiotics (2nd infection)
Eczema (AD) immunomodulators pimecrolimus(Elidel)1%, tacrolimus, protopic.
psoriasis topical corticosteriods, tar prep topical/bath/soak Estar, PsoriGel. Retinoids tazarotene(Tazorac)gel. Biologic response modulators entanercept(enbrel), methotrexate(MTX), cyclosporine(Neoral)
Acne Vulgaris benzoyl peroxide cream,lotion,gel,wash. tretinoin (Retin-A, retinoic acid) cream gel liquid. Top antibacterial clindamycin,erythromycin, metronidazole, azelaic acid, benzoyl peroxide + erythromycin (Benzamycin). benzoyl peroxide+glycolic acid.
Acne Vulgaris Systemic antibiotic po tetracycline, erythromycin, minocycline, doxycycline->severe OCP...decreases adrogen production. isotrentinoin(Accutane)-->generic forms only
