Scavenger System

What is the purpose of the scavenger system? Collection and removal of excess gas in the OR; The volume of gas that enters the circuit must equal the volume leaving the circuit
What are the 5 components of the scavenger system? Gas Collecting Assembly; Transfer Means; Scavenging Interface; Gas Disposal Assembly Tubing; Activ or Passive Gas Disposal Assembly
Describe the Gas Collecting Assembly It collects excess gas and delivers it to the transfer tubing;
Where does the gas collecting assembly collect gas from? APL valve and ventilator relief valve
Describe the Transfer tubing Carries gas to the scavenger interface (either 19 or 30 m ID rigid tubing for saftety purposes); usuallyu color coded; There are 2 separate tubes (APL and Vent relief valve)
Describe the Scavenger Interface System Most important part of the system; positive relief valve is mandatory; may be an open or closed system; connection between the pt circuit and the hospital suction system
What is the purpose of the pressure relief pop-off valve? Protects the circuit from excessive + or - pressures;
Pop-off Valve parameters in Closed System If pressure from the circuit exceeds 5cm H20 the valve opens; If the reed valve is heard, suction needs to be increased to relieve pressure If the negative pressure from the suction is -0.25 to -1.8 cmH2O, the valve opens and sucks air from outside in.
Which system utilizes an expandable bag (open or closed)? Closed
What is the purpose of the bag in the closed system? The bag expands to account for pressure changes during exhalation; If the bag is over-inflated, suction needs to be increased to relieve pressure.
Explain How the Open Scavenger System Works The heavier anesthetic gas remains at the bottom and is removed by the hospital suction; (suction is required, active disposal)
Describe the gas disposal assembly tubing Conducts waste gas from interface to gas disposal assembly
What are the 2 types of gas disposal assemblies? Active and Passive
Describe active gas disposal assembly Utilizes negative pressure system (vaccum); can be used in open or closed systems
Describe passive gas disposal assembly Positive pressure from the circuit and the ventilator forces the waste gas through the scavenger system; can only be utilized with a closed system.
What do you do if you loose suction in a closed system? Increase gas flow rates to force waste through the scavenger system. The active closed system that looses suction simply becomes a passive closed system.
What do you do if you loose suction in an open system? Stop the gases immediately and switch to Total IV Anesthesia (TIVA).
Which gas disposal assembly is safer for OR staff and why? The closed system is safer for the OR staff because the waste gas is contained and the staff are never exposed to it (except if pressure builds up and the pop-off valve opens))
Which gas disposal assembly is safer for the patient and why? The open systems are safer because there is minimal risk for increased pressures.
Which anesthetic gas is related to birth defects? N2O
NIOSH recommendations for N2O exposure 25 ppm
NIOSH recommendations for halogenated gas 2 ppm
NIOSH recommendations for combination of halogenated gas and N2O 0.5ppm
How can you prevent gas leaks? Check scav sys daily; Ensure tight mask fit on pt; Only turn gas on when connected to patient; Use Low Flows; Wash out tubing with fresh gas; proper inflation of ETT cuff; Don't spill agent; disconnect lines at end of day; turn off tanks at end of day
In a closed system, what is an indication of too much suction? The bag will remain deflated
In a closed system, what is an indication of not enough suction? The bag will become overinflated and the reed valve will be heard which is an indication of barotrauma.
What adjustments are necessary when waking a patient up? Turn off anesthetic agent and turn up fresh gas flows and suction.
What adjustments need to be made to the scavenger system if you increase the patient's TV? Increase suction away from scavenger system to prevent barotrauma.
