Lithium MOA thought to affect many neurotransmitter functions. It probably corrects an ion exchange abnormality in the neuron and normalizes NT functions.
Lithium treatment stabilze mood in Bipolar disorder. Prevents and treat manic episodes.
Lithium therapeutic range 0.5-1.5 mEq/L
Lithium notes draw levels before A.M dose,check levels weekly then every 2mo, monitor weight, stable consistant NA intake, fluid and electrolyte balance, mental status, assess for toxicity. Use cautiously in elderly, thyroid disease, diabetes.
Lithium SE and toxicity fine hand tremors, weight gain, subjective feeling, ofmental dullness, polyuria, GI discomfort, mild nausea, thyroid dysfunction.
Lithium Early Toxicity Nausea/vomitting, coarse hand tremors, muscle weakness, diarrhea, slurred speach.
Lithium Severe Toxicity Mental confusion, fever, seizures, cardiac dysrythmia, coma, muscle irratibility and decreased coordination, decreased urinary output, severe hypotension, death
valproic acid (Depakene) anticonvulsant mood stabilizers
sodium valproate (Depakon) anticonvulsant mood stabilizers
divalproex sodium (Depakote) anticonvulsant mood stabilizers
carbamazepine (Tegretol) kindling effect anticonvulsant mood stabilizers
lamotrigine(lamictil) anticonvulsant mood stabilizers
anticonvulsant mood stabilizers MOA decreased nerve cell irratibility (less likely to conduct impulses), increase in GABA, increase in post synaptic membrane response to GABA and reduced calcium influx into nerve cells.
anticonvulsant mood stabilizers treatment clinically effective in treating symptoms of mania and in preventing reoccurence of episodes.
anticonvulsant mood stabilizers SE transient hair loss, tremors, thrombocytopenia, weight gain, GI upset
kindling process process of small seizure activity that builds up into major seizure or manic episode.
Carbamazepine can cause bone marrow suppression with decrease number of WBC. Clients need WBC count assessed.
