Question | Answer |
---|---|
What are antiulcer drugs? | Antacids, H2 Blockers, PPI (Proton Pump Inhibitors), Pepsin Inhibitor, Prostaglandin Analogue |
Where does most drug absorption occur? | In the Duodenum |
What is absorbed in the stomach? | Lipid soluble drugs and alcolhols |
What do glands secrete? | Mucus, Pepsinogen, Bicarbonate, HCl, Intrinsic factor, Gastrin, Serotonin, Hormonal products |
Gastrin hormone | Gastrin producing cells |
HCl acid, Intrinsic factor, Needed for absorption of Vitamin B12 | Parietal Cells |
Chief Cells break down protein | Pepsinogen - pepsin (enzyme) |
Mucous producing cells | mucous |
Histaminestimulates production of HCl | Enterochromaffin-like (ECL) cells |
Somatostatin (inhibitory) | D-Cells |
D-cells | at the end of the digestive cycle to reduce acids |
Gastrin produces what? | Hcl and pepsinogen |
Vagus nerve is stimulated by what? | sight, taste, smell |
food in stomach causes what to be released? | Histamin |
What completes the digestion of fats, carbs, and protein? | hormones, bile, and pancreatic acid (trypsin, chymotrypsin, lipase, and amylase) |
What causes a peptic ulcer? | H. pylori,Extreme physiological stress (chemp pts), NSAIDS, Zollinger-Elsin Syndrome (ZES), PUD (peptic ulcer disease) |
Antacids | Neutralize gastric acid by increasing the pH in the GI tract, reducing pepsin activity |
Antacids | Excreted in feces (some in urine, breast milk) |
Antacids | Amphojel, Maalox, Mylanta, MOM, Tums |
Antacids | Used for indigestion, heartburn (r/o GI vs Cardio), GERD, PUD, hyperphosphatemia |
MOM | Mg Hydroxide, Diarrhea, Excreted in milk and urine |
Maalox, Mylanta | Al & MgBalanced |
Amphojel | Al hydroxide, Binds dietary phosphate, Al intoxication, Constipating |
TUMS | Ca CO4, Binds dietary phosphate, Milk-alkali syndrome, (Ca OD) |
What is a Ca OD? | urinary problems, confusion, hypercalcium, renal problems |
Antacids | Decrease absorption of quinolones, iron, isoniazid, phenytoin, and digoxin - administer 2 hours apart |
Antacids | Causes premature dissolution of enteric-coated tablets - Administer 1 hour apart |
Antacid Teaching | Assess Electrolyte imbalances, changes in elimination |
Antacid Teaching | Take 1-3 hours after a meal, shake suspension well, drink water after chewing, give 1 hour apart from enteric coated pills, heart failure, HTN - so check sodium content |
H2 Blockers | Inhibit gastric acid secretion by inhibiting the action of Histamine at Histamine2 receptors in gastric parietal cells |
H2 Blockers | Cimetidine (Tagamet); Famotidine (Pepcid - Q12 hours); Ranitidine (Zantac) |
What are the indications for H2 Blockers? | GERD, UGIB, ZES Syndrome, PUD |
H2 Blockers are contraindicated when? | W breast feeding (cimetidine tagamet) |
H2 caution | pregnant, elderly, impaired renal and hepatic function |
H2 s/e | HA, dizziness, confusion, mild disrrhea, rash |
H2 Blocker Cimitidine (Tagamet) | crosses placenta barrier and found in breast milk |
H2 Blocker Cimitidine (Tagamet) | May increase serum levels and toxicity of benzodiazipines, ca cahnnel blockers, tricyclic antidepressants, procainamide, lidocaine, oral anticoagulants, phenytoin, propranolol, quindine, and theophylline |
H2 blocker given with Carmustine may increase risk of bone marrow toxicity |
