Parenteral Injection

Parenteral route refers to... intradermal, subcut, IM, & IV
Aspirate is used... in IM injections. prevents injection into blood vessel.
Aspirate (technique) After insertion of needle, pull back slightly on plunger of syringe.
Angle of insertion for intradermal injections 10-15 degrees
Gauge used for intradermal injection smallest avaliable (book: 26-29 gauge)
Sites for intradermal injection forearm, upper back
intradermal maximum injection Manuasa: small amount (book 0.01-0.1mL)
Technique for intraderma injection: hold skim taught, bevel up insert needle. Don't aspirate, don't massage.
Subcut angel of insertion 45 or 90 degrees. (depends on technique and size of patient)
subcut needle gauge 25-30
subcut needle length 3/8 or 5/8
subcut maximum injection Manuasa 1 ml or less (book up to 2 mL)
subcut sites upper outer arm, anterior thigh, abdomen, upper back, buttocks
subcut injection technique: bunch or stretch; skin gloves are optional; massage (but not with insulin, heprin or heprin like prodcuts)
does subcut injection allow administration of irritating substances? NO
IM injections medication injected into muscle
IM, okay for irritating substances?? Yes
IM angle of insertion always 90 degrees
IM needle gauge 20-22
needle length for IM 1 1/2 - 3 inches (1" for deltoid) Can depend on size of patient.
Main adult sites for IM injection Dorsogleuteal, ventrogleuteal, vastus lateralis
maximum injected w/ IM 3 mL except w/ deltoid which is only 1 mL
