what is the GCS | Glascow coma scale - measures how comatose a patient is |
what are the components of the GCS | eye response, verbal response and motor response |
how high and low does the GCS go | 3 is the lowest meaning comatose and 15 is A&O x4 |
what are the components of eye response | 1. no response 2. responds to pain 3. respond to speech 4. spontaneous eye opening |
what are the components of verbal response | 1. no response. 2. inaudible word mumbling 3. inappropriate words that are nonsense. 4. confused wording. 5. full normal appropriate sentences. |
what are the components of motor response. | 1. no response 2. abnormal extension towards sensation. 3. abnormal flexion (touch right withdraw left) 4. withdraws from pain. 5. moves to localized pain. 6. obeys motor command. |
what value is considered a critical GCS score | 8 or less |
define expressive language | read written word, verbally identify objects |
define receptive language | follow verbal commands |
define dysphasia | thoughts cant be turned into spoken word |
define dysarthria | can speak but trouble verbalize the words correctly |
describe light touch assessment | have patient close eyes and touch body parts lightly and have them identify |
describe pain sensation assessment | have patient identify what is a dull or sharp pain when touching body parts. |
describe kinesthetic sensation assessment | patient with eyes closed move body part and have them identify where it is in space. |
describe upper fine motor skills assessment | finger to nose, finger to patient nose with eyes clsoed |
describe lower finer motor skills assessment | heel to shin test, heel to toe test |
how could you assess balance | have them stand one one leg |
how do you assess patient gait | have them walking upright looking for posture, sway, arm movement, foot placement. |

describe snout reflex results | positive - lips pucker (abnormal in adults) negative - nothing (normal in adults abnormal in babies) |
describe corneal reflex results | positive - eye blinks and waters (normal) negative - no eye response (abnormal) |
describe babinski reflex results | positive - toes fan outward (abnormal) negative - toes clench inward and down (normal). |
describe patellar reflex results | positive - leg extends (normal) negative - no leg extension (abnormal) |
describe the scale on which reflexes are graded | 0- no response 1- mild response 2 - medium response 3- large response 4 - repeating response |
what are considered abnormal scoring for reflexes | 0 - no response and 4 - repeated response |
what is recent memory | the ability to remember over the last few minutes |
what is immediate memory | the ability to remember over the last few days |
