What is a straight Cath? In and out, single use, drain bladder 5-10 minutes then remove. Single lumen
What is another name for a straight cath? intermittent
When would you use a straight cath? Used if pt. can’t urinate, assessment of residual urine (cath right after they try to void); cath to relieve pressure or discomfort due to distention; urine specimen
What is a Coude? pointed tip, intermittent catheterization. Sometimes used for enlarged prostate
What is a Continuous Urethral Catheterization (aka Indwelling)? Inflatable balloon anchors catheter in place. 2 or 3 lumens – urine, balloon inflation, meds/fluids
When would an indwelling cath be used? - Used for obstruction to urine flow, surgical repair or bladder, urethra, surround structures, gyn. Surgery, prevention of obstruction, - Hourly output measurements, - Continuous intermittent bladder irrigation, short term and long term
When would an indwelling cath be used for short term? orthopedic client that cannot be safely turned and repositioned, when frequent moving causes pain, to empty the bladder during childbirth
When would an indwelling cath be used for long term? severe retention with UTIs, Skin breakdown, Terminal illness
What is the purpose of Suprapubic Catheterization? urethral trauma, long term cath – sexually active, following pelvic/urological surgery, some gynecological conditions, long term catheterization for incontinence, intolerant of urethral catheter, young or infant boy
What are some of the problems with Suprapubic Catheterization? Prone to poor drainage due to mechanical obstruction of the catheter tip by the bladder wall, sediment, and clots
How is a suprapubic cath inserted? Inserted by physician through abdominal wall above the symphysis pubis into bladder. Tape on abdomen
How is a suprapubic cath anchored? Anchored by sutures or seal or water seal balloon
What type of anesthetic is used to insert a suprapubic cath? Local or general anesthetic
How is the suprapubic cath cared for? Care is similar to urethral
What are the 3 types of Types of female supra catheters? The Banono, Silastic Suprapubic Cystocath and the The Rutner Percutaneous Suprpubic Balloon catheter
In addition to privacy and gloves describe the procedure for application of a condom catheter? - Wash penis with warm soapy water, Encircle partly with the adhesive strip tape, Gently press condom to adhesive strip and - Attach to drainage bag
What should you assess for after application of a condom catheter? for constriction
How is a condom catheter cared for? Remove daily & clean
Define bladder irrigation Bladder irrigation is the process of flushing the bladder with normal saline to prevent or treat clot formation. Bladder irrigation may also be used to instill medications such as antibiotics for treating bladder infections.
What is the purpose of bladder irrigation? Removal of clots, mucus, urine after GU surgery, maintain patency of catheter
What is a common surgery which causes a need for bladder irrigation and why? surgery on the prostate may result in bleeding into the bladder. Therefore when a client has had surgery on the bladder or a transurethral resection of the prostate, the indwelling catheter will need to be irrigated
What is the purpose of irrigating the bladder after prostrate surgery? to maintain its patency by preventing the formation of blood clots or removing those that do form.
What is the purpose of a Triple Lumen Catheter? Provides homeostasis, Facilitates urinary drainage and Provides port for irrigation
What size is the Triple Lumen Catheter? 18-22
What is the purpose of the first lumen of a triple lumen cath? 1. Balloon – 30 ml – Keeps catheter inside bladder, provides homeostasis
What is the purpose of the second lumen of a triple lumen cath? removes urine into closed drainage system
What is the purpose of the first third lumen of a triple lumen cath? connects to sterile irrigation solution (helps remove mucus, clots, tissue & can insert med)
What is a closed irrigation system? Uses a three-lumen indwelling catheter
What are the advantages of a closed irrigation system? reduced risk for infection
What are the disadvantages of a closed irrigation system? the patient is attached to multiple tubes and the bag of irrigation fluid.
What data is required for documentation of a cath insertion? Type and size of catheter, Amount of urine obtained, Description of urine, Disposition of specimen, if obtained, Patient response to procedure, Time
What size cath would you use with Children:? 8 – 10 Fr. 3 ml
What size cath would you use with Women? 14 – 16 Fr.
What size cath would you use with Men? 16 – 18 Fr
What size balloon would you use with children? 3 ml
What size balloon would you use with adults? 5 – 30 ml
What size balloon would you use with a Triple lumen? 30cc
What would happen if you used normal saline for the cath balloon? It may crystallize
What would you assess on a patient with a cath? - Color, clarity, amount, Fluid intake, Temperature, Comfort, Site
What are 4 nursing cares that need to be done on a patient with a cath? - Force fluids, Examine sight, Dressing change, Strict aseptic technique
What are Interventions to maintain patency of the cath? Prevent kinking, maintain gravity for drainage, Have client turn from side to side, Milk tubing, May have to irrigate (need MD order)
What are Bladder Spasms? involuntary, sudden, violent contractions of the muscles of the bladder that can be prolonged and painful.
When can bladder spasms occur? Can occur as a side effect of catheterization or secondary to surgery on the bladder or urethra.
What drugs can be used to reduce bladder tone and relieve spasms? - Antispasmodics and anticholinergics
What can be given to relieve the pain of Bladder Spasms? Narcotics
What are two interventions besides drugs that can be used to relieve bladder spasms? Manipulating the catheter to move the balloon away from the neck of the bladder may alleviate the spasms. Increasing fluid intake may be helpful as well.
What is a - Transurethral resection (TUR) ? Resection of prostate: Partial removal of prostate, Removal of medial lobe
How is a Transurethral resection (TUR) done? o Resectoscope passed thru urethra to excise & cauterize prostate tissue
What height should the solution be hung for bladder irrigation? 2-3 feet above client

Describe the Silastic Suprapubic Cystocath? - End of tubing connected to straight catheter tubing, - Stopcock present, - Plastic disc is glued to client’s abdominal area to secure catheter
Describe the Rutner Percutaneous Suprpubic Balloon catheter - Balloon catheter is inserted into the bladder thru a small slit (3-4 mm), is anchored in the bladder with sterile saline in the balloon, may be secured on the ab skin by strip of tape, - One way stopcock is present to direct flow of urine or to stop flo
How much sterile saline is used in the balloon of a Rutner Percutaneous Suprpubic Balloon catheter? 2.5-3 ml
What is done with the excess tubing of a Rutner Percutaneous Suprpubic Balloon catheter? Tape excess tubing to abdomen so flow of urine is not obstructed
