What can be observing for the documentation as the P.S.S? Vital signs, weight gain/loss, skin color, temperature, moisture, appetite, level of activities increase/decrease, complains of pain, ambulation, breathing, mood, realtionship with others, behavior, hearing, vision, self-care ability, & symtoms.
What are vital signs? Measure of pulse, pain level, blood pressure, and respotry.
What also you have to look out for at skin? Any brusies, cuts, lumps, or rashes.
What is mood? If you are happy/sad/anger/frustrated/etc.
What symptoms there are? Any person conplains of a headache, sewelling anywhere in body-especailly in feet and legs, sore throat, etc.
What is a objective observation? observation is one that is measuravle or factual in nature, for example a consumer temptures, sometimes you have to figue it out on your own for the consumer. And you yousing all your senses.
What are all your senses doing objective observation? Sight: Any you see blood in urine or on body, serlling, or bruises. Hearing: If you hear strage breathing, crying, or couthing. Nose: If you smell urine or body order. & Touch: If you fell any lumps under/above the skin,change in pulse,&temputure(skin).
What is subjectve observation? A statement or complaint from a consumer about symptomes that only he/she can describe. For example "I have a headache."
You must report_______! Imenetly.
What are the rules when you do in a report? Be complete and detailed, do not draw conclusions; report only the facts, if written make sure it's neat and easy to read, & never use meducak terms or abbrecuatuions unless you are sure the meaning and apporiate for your audience.
What are incident reports? Documents that record any unsual happenings. Any consumer that fight, injury, missing poseesions, cosumer runs away(elopement), meddication error & yourself injury.
What is it called the P.S.S must fill a card that record there time in/out? Time card.
What are the rules for any documention? (There are 12 rules, I can't fit it all in this card see book on pg 92)
Make sure you put a note of ___ of care or treatment in documention, make sure you try at least three or more times befor you note it. refusal.
Always maintain _______ for the consumer documention. No frieds, family, or vistors must see the documention. Confidentiality.
