What can be done to ensure the safety of an infant? never leave unattended, use crib rails, check for choking hazards, *car seats*
What are some safety measures that can be taken for preschoolers/toddlers? Childproof the home, prevent poisoning, be alert for abuse, maintain proper car seat usage
For school aged children what is important to teach them about? stranger-danger, bicycle safety, seat beats
What are some signs of possible child abuse? history of unexplained injuries, thermal burns, bite marks, nightmares, excessive sexual curiosity, fear of strangers, decreased school attendance
What are things important to teach adolescents concerning safety? driving skills, avoidance of tobacco and alcohol, guns and violence, and sex education
What are some safety considerations for adults? domestic violence, stress related effects, unsafe drug and alcohol use
What is intimate partner violence? domestic violence or battery cause by a boyfriend, spouse, or girlfriend
What are some safety considerations for older adults ? orient to surroundings, maintain vehicle in good working order, use medication trays (pre-fill and hide rest)
What is the leading cause of accidental death in older adults? falls
What are restraints? physical devices used to limit a persons movement
What is a sentinel event? an unexpected death or injury either physical or psychological
What is true about people of small stature and the safety of side rail use? they are more likely to slip between the rails
what must a nurse have in order to use restraints? a written doctors order
How often does a restraint order need to be evaluated and rewritten? every 24 hours
What are 4 examples of restraints? mummy restrains, elbow restraint, vest restraint, wrist/ankle restraint
What are some physiological hazards associated with restraints? suffocation from entrapment, impaired circulation, altered skin integrity, fractures, aspiration or breathing difficulties, incontinence, changes in mental status
Who set up the sentinel event advisory group? the Joint Commission on Accreditation of Healthcare Organization (JCAHO)
What organization regulates all hospitals through the government? JCAHO
What does the acronym RACE stand for? Rescue (ppl. In immediate danger), Activate (emergency response system), Confine (the fire), Evacuate (patients in danger)
What is a confidential document that objectively describes the circumstances of an accident called? an incident report
Where/when should an incident report be filled out? immediately after the accident, effects should be recorded with in the medical record not the incident report itself
What are some ways to prevent equipment related injuries? use only as intended, use with care, only use appropriate plugs, do not twist cords, be alert of faulty equipment and wet surfaces
What are a few of JCAHO safety goals? improve overall safety, improve proper identification, improve medication safety, eliminate wrong site, wrong patient, wrong surgery incidents, reduce risk of healthcare acquired infections
What are 2 ways to properly identify patients? verbally and by verification of wrist bands
What is it called when there is deliberate spread of pathogenic organisms into a community? bioterrorism

What is one way to help older adults stay active in a long term facility rather than having to restrain them? place a picture of themselves at the doors to their rooms to help them find their rooms
