What is Assessment in Nursing Process? the 1st phase in NP; is the sytematice & orderly analysis of data about the past/present health status of the client/patient for the purpose of making the nursing diagnosis.
What are the assessment guidelines? 1) Biographical & Demographic data 2) Health History including family 3) Subjective & Objective data about current health status 4) Generic or Folk treatments 5) Social, Cultural, Spiritual & Environmental data 6) Behaviors
What is Nursing Diagnosis in NP? 2nd phase in NP, (Nanda) a clinical judgment about individual family or community responses to actual or potential health problems; statement identifies an actual or potential health problem or area of concern that may be amenable to nursing actions.
How should Nursing Diagnosis be ranked & based on? Nursing Diagnosis are IDENTIFIED and they should be RANKED IN ORDER OF PRIORITY based on input from both the client and the nurse.
What is Outcome Identification? 3rd phase in NP; the nurse & client set realistic & measurable expected outcomes, w/c are derived from the diagnosis; the client & his/her family should be consulted before formulating the client's outcome, w/c should be realistic & attainable.
What is Planning in NP? (2) 4th phase; can be described as the determination of what can be done to assist the client & reflects nursing actions; involves the devlpt of strategies to attain specific expected outcomes to resolve actual or potential health problems.
What is Planning in NP? (1) deals to both actual/potential problems & for reaching identified outcomes in an orderly fashion; provide means for organization, giving direction/meaning to the nursing actions used in helping the client/family to resolve health problems.
What is Implemetation? 5th phase in the NP; occurs when the nurse implements the actions identified in the previous phase; putting the plan into action; other terms used: ACTION or INTERVENTION.
What is the difference between IMPLEMETATION and INTERVENTION? IMPLEMENTATION refers to putting a plan into action. INTERVENTION speaks to involvement in the affairs of another, a coming between the other and a problematic situation.
When do Implementatin begins? it begins when the nurse selects those actions most suitable to achieve the identified outcomes.
What are the 7 categories of Nursing actions? (CCampbell) 1) assertive 2) hygienic 3) rehabilitative 4) supportive 5) preventive 6) observation 7) educative
What do nurses need to be clear about in their functions in doing nursing actions? 1) dependent 2) independent 3) interdependent
What is important to consider in planning Nursing actions? it is important to consider the CLUSTER of CONSEQUENCES of both POSITIVE & NEGATIVE VALUE that can be EXPECTED to OCCUR with and following each action.
What is Evaluation in NP? 6th and final NP; the clients progress toward attainment of outcomes are evaluated; the key to appropriate evaluation of nurse-client actions lies in the planning phase of the nursing process; it may lead to REASSESSMENT.
What are the 5 steps that Evaluation should consist of? (Wilkinson) 1) Review 2) Collect 3) Compare 4) Record 5) Relate
What should REVIEW consist of in Evaluation? the stated (predicted)outcome
What should COLLECT consist of in Evaluation? data about the clients responses to nursing interventions
What should COMPARE consist of in Evaluation? the actual outcomes to predicted outcomes & decide if goals have been met.
What should RECORD consist of in Evaluation? the conclusion
What should RELATE consist of in Evaluation? nursing plans & intervention to client outcomes
What are the 2 types of Evaluation? 1) Structure evaluation 2) Process evaluation
What is Structure evaluation? relates to such things as appropriate equipment to assess the client or to carry out the plan & to record evaluation conclusions.
What is Process evaluation? focuses on the activities of the nurse, can be done during each phase of the nursing process, or it may be carried out at the end of the process.
