What are nursing management to abd ultrasound? advise pt drink 1-2 quarts of fluid prior to fill bladder to lift and stabilize uterus, wedge under rt hip
What are nursing management to transvaginal untrasound? assist into lithotomy position, lubricate probe with gel
What is nonstress test (NST)? test for fetal well-being with transducer and tocotransducer.
What does nonreactive NST indicate? fetal heartbeat does not accelerate adequately with fetal movement or no fetal movements occur in 40 min. Anticipate CST and/or BPP.
What is vibroacoustic stimulation? sound source activated over maternal abd to awaken sleeping fetus
What is contraction stress test (CST)? stimulate contractions and determine how fetus will tolerate stress of labor
What is a negative CST? normal finding: within 10 min period, there are no late FHR declerations with 3 contractions
What are risks with CST? hyperstimulation of uterus, preterm labor (monitor contractions that are >90 sec and/or 2 min in frequency)
What is biophysical profile (BPP)? uses ultrasound to assess fetal well-being in five areas
What are the 5 areas in BPP assessment? reactive FHR, fetal breathing movements, gross body movements, fetal tone, amniotic fluid volume (normal score:8-10)
What is amniocentesis? test for chromosomal abnormalities, infection, and lung maturity performed after 14 wk
What are high and low levels of alpha-fetoprotein (AFP) associated with? High: neural tube defects, anencephaly Low: down syndrome, mole pregnancy
What do fetal lung tests include? 2:1 Lecithin/sphingomyelin (L/S) ratio = fetal lung maturity, phosphatidylglycerol (PG) absence linked with resp distress
What are nursing management for amniocentesis? informed consent, empty bladder, locate placenta with ultrasound, cleanse abd with antiseptic, administer RhoGam if pt Rh-, have pt rest 30 min after procedure
What are risks of amniocentesis? amniotic fluid emboli, hemorrhage, Rh isoimmunization, infection, inadvertent fetal damage, PROM
What is percutaneous umbilical blood sampling (PUBS)? common method for fetal blood sampling and transfusion. (Can also detect chromosomal anomalies)
What is chorionic villus sampling (CVS)? Can test for chromosomal anomalies earlier than amniocentesis, done at 10-12 wk
What are nursing management for CVS? same as amniocentesis except fill bladder b/4 procedure and assist in lithotomy position for transvaginal probe
