What Exactly is Long Term Care? care that is long, doesn’t just begin in a nursing home, begins when cant do things anymore that you used to, need help from here on out. Family & friend support is also a type of long term care.

What do Geriatric Care Mangers do? Geriatric Care Managers – new nursing specialty. Corrdinate long term care. Set up the services that people need to be able to stay in their homes.
Only 1 out of 5 who need them use community-based services! Why? don’t know about them. Pride is another reason people don’t want them. Example….meals on wheels, how to get to grocery store delivery.
What are some barriers that may result in OAs not using community-based services? Barriers: Cost...may be too expensive. Impersonal…don’t want someone helping that is not family. Many people except their family to take care of them.
Medicare may pay for some home health care. 3 day hospitalization required for people to get home health care to be covered. Have to have documented improvements in order to get home health….Give some documented ex? wound getting smaller, activity getting better.
All other home health is ____? self paid. Gets really expensive – 19 $/hr, more at night. 11,000 dollars a month. So may not be cheaper to stay at home rather than in a nursing home.

What does aging in place mean? Aging in Place – they want their house, their classes, their pets, their visitors. Basically they want to age were they have always resided.
When a person can no longer live alone – what happens then? They get home health, Independent living; assisted living; skilled care; long term nursing home care; hospice; usually until they die
Life Care/Continuing Care Retirement Communities…Independent Living – Assisted Living – Skilled Care – Long Term Nursing Home Care…Get needs met until you die & involves ___ & is generally ___? Involves moves, but only short distances – each move is traumatic. Generally very expensive, sometimes require large “buy-in” up front and additional monthly fees

What is the Eden Alternative? a Nursing Home with regular interactions with kids, pets that live on site, and are involved in taking care of them. Gardening. Family atmosphere that make family like decisions such as naming a pet, planning party, or going to a party.

Other Creative Options for OA living is Green Houses (not in text); what is it? same as sorority or frat. Group of people that share house, and relish good food with good friends. Bake & cook. Can improve health and is cheaper than nursing home care.
