Ergot Alkaloids

DrugsMOA & ADRs
Ergotamine MOA: 5-HT 2 antagonist autacoid (local hormones), drug of choice for moderate to severe migraines, best if used in the prodromal phase (just beginning); ADRs: don't use in pregnancy, peripheral vascular disease & CAD, causes n/d & severe vasospasms
Methysergide MOA: 5-HT2 antagonist autocoid (local hormone) used in prevention of migraine, usually given during prodrome thus only prophylactic, not intended for use during attack; ADRs: nausea, diarrhea & severe vasospasms
Bromocriptine MOA: inhibits prolactin release used for hyperprolactinemia (pituitary adenoma) & infertility
Sumatriptan MOA: 5-HT 1 agonist, more powerful than the ergot alkaloids, useful in prevention and treatment of ongoing migraine
Cyproheptadine MOA: block 5-HT2 receptors that used in the prevention of migraines
Odansetron MOA: 5-HT3 antagonist used as a powerful anti-emetic
Granisetron MOA: 5-HT3 antagonist used as a powerful anti-emetic
Metoclopramide MOA: 5-HT4 agonist used as an anti-emetic causing increased GI motility
Buspirone MOA: 5-HT1 agonist used for anxiolysis
